WHILE HITTING THE TOOTH PIECE OF METAL, THAT APPEAR ED TO COME FROM THE TOOTH, FLEW OFF AND PENETRATED TO THE BONE, THE EMPLOYEE S LEFT ARM BETWEEN THE SHOULDER AND ELBOW.
EMPLOYEE WAS USING A HAMMER AND CHISEL WHILE DOING MINOR REPAIR WORK ON A BULLDOZER WHEN HE STRUCK HIMSELF ON THE BACK OF HIS HAND WITH THE HAMMER.
