Showing all 3Employee was opening a bag of floor dry with the help of the injured worker. The blade he was using slipped, went through the glove of the injured worker and cut his thumb. He received three stitches.
Worker was flopping the gate on the #5 tube to go to the #5A tube when the come-a-long chain came off and hit him in the lip. He was cut and recieved stitches. Returned to work. No impairment.
