When setting up to pull belt off EE dropped the belt pulling arm on left foot just behind the steel toe. The doctor's report said nothing broke but told EE to wear a walking boot, ice it and rest it. EE has been sweeping floors and cleaning outside since that date.
While cutting piece of rubber belt the knife came out and struck employee in forearm resulting in 5 stitches.
Knocking a block out of a wall another block fell out and struck EE on the left hand. Went to doctor and came back with NO restrictions, small fracture on left hand pointer finger, wearing a splint for a couple days. No work missed.

Installing a 6 foot bolt with a strap. The bolt began to bend over, the individual stopped to straighten the bolt by striking it with a wrench. The strap slid down the bolt while EE was swinging the wrench and EE struck hand on the strap, lacerating EE's fingers.
Miner was cleaning coal from a chute with a scraper when ee lacerated right hand
Victim was tightening a bolt on the bolter, the wrench slipped off the head of the bolt, causing their hand to hit the bolter, cutting the top of their hand. Employee did receive 4 stitches. No work lost.
Individual was helping re-bolt an area of the mine that was already bolted when the helper on the bolter drill steels was getting stuck in the roof. The individual used a wrench to hold pressure on the strap so the helper could get the steels out and got a finger pinched between the steels and the roof.
While changing dolly wheels on an MBC EE felt something in the right shoulder. While swinging a sledge. Had surgery on 12/16/16 to repair shoulder.

Employee was working with a 1/2 drive ratchet and a socket.The socket slipped off of the bolt and he drove his hand into the bottom breaking a small bone in his hand.
