WHILE TIGHTENING THE BOLTS ON A HYDRAULIC HOSE OF THE 922B ENDLOADER, MECHANIC I.NAME WAS LYING HEAD DOWN IN THE CENTER OF THE ENDLOADER. DURING THIS TIGHTENING PROCESS I.NAME WAS PULLIN G UPWARD ON THE RACHET HE WAS USING WHEN THE RATCHET SLIPPED OFF OF THE BOLT.
