EE stated he was replacing conveyor chain on a joy 21sc shuttle car using a com-a-long. Chain broke on the com-a-long causing ee to fall backward, causing a possible strain to ee back and neck. EE started missing work on 4/14/11.

EE stated he was placing a conveyor belt tail roll in place. Come-a-long chain came loose- caught both hands between tail roller & steel plate. Fracture to little finger left hand, ring finger, r. hand, both fingers required stitches from lacerations. Started missing work 6/8/07.

Employee stated he was using a sledge hammer to remove a pin holding boom onto roof bolting machine. A piece of steel from pin struck him in upper left arm. He missed one shift. Returned to work on 5-16-07.
