On 1-9-2006 employee was loading holes at a shot location and injured left shoulder. The next day the employee was using a pry bar on a drill steel and injured right shoulder. MRI was performed and Doctor diagnosed rotator cuff injury of both shoulders. Reported information to employer on 1-25-2006. First lost day was 1-11-2006.

EMPLOYEE WAS USING A SLATE BAR TO CLEAN THE TRACKS ON THE KOMATSU PC 4000 SHOVEL. WHILE PULLING ON THE BAR HE PULLED A MUSCLE IN HIS RIGHT SHOULDER.

EE WAS PULLING ON A WRENCH WHEN HE FELT SOMETHING POP IN THE LITTLE FINGER OF HIS RIGHT HAND. HE FINISHED HIS SHIFT AND WENT TO THE EMERGENCY ROOM THE NEXT DAY WHERE HE WAS DIAGNOSED WITH A BR OKEN FINGER.
