Employee was removing a bushing from a shaft and the bushing quit moving. He then used a screwdriver to pry on the bushing when it suddenly broke free and slid down the shaft and pinched his finger between the bushing and the manlift he was standing in causing cut to his right index finger. He was taken by private vehicle to the emergency room for evaluation and received 1 stich.
Employee attempted to loosen fitting on dozer air conditioner accumulator and pulled muscle in his left shoulder.
Employee received a laceration which required stitches to his right index finger, from the lid on a can of soup while attempting to open it with a pocket knife.
Employee was welding the floor plate on Komatsu truck box. While he was chipping slag off of a welded area with a chipper, he felt something go into his right eye underneath his glasses and face shield. Seen by MD and had a small piece of metal removed from eye.
Employee was tightening a head bolt on a drill using an allen wrench and hammer. Hammer bounced off of steel rack hitting employee's knuckle of his left index finger causing a laceration that required stitches.
Employee lacerated left thumb with razor knife while opening a box of welding rod which required sutures.
Employee was using utility knife with new blade to cut 1/2" poly pipe when knife cut through pipe and knicked inside part of employee thigh. Employee required stiches for cut.
Employee was tightening bolts on piller block when his wrench slipped, pinching his left index finger between wrench and toe board causing a laceration to employees left index finger requiring sutures.
Employee was attemting to repair a headlight problem on unit 3056. While in the process of cutting the boot off the back of the headlight, his pocket knife slipped and cut the side of his left thumb. Laceration required stitches.
While tightening nut on 789 left rear dual pilot stud, torque wrench pinched right hand between torque gun and inside of wheel rim. Seen in ER and placed on restricted duty for soft tissue injury to the palm of right hand.
EMPLOYEE WAS STRUCK IN THE ARM WITH A SLIVER OF METAL WHICH ORIGINATED FROM THE REMOVAL OF A BEARING RACE WHICH WAS BEING WORKED ON APPROX. 8' AWAY USING A DRIFT PUNCH. EE WAS SEEN BY MD AND THE SLIVER OF METAL WAS REMOVED FROM HIS FOREARM.
EE WAS USING A LIFTING CABLE WITH SHAKE OUT HOOKS TO REMOVE PLANKS FROM LOWBOY DECK. EE HOKED 1 CABLE TO A PLANK, OTHER 3 CABLES WERE WOOSE AND NOT SECURED. ONE LOOSE CABLE CAUGHT ON BEAM AND WHEN EE TIGHTENED UP HOOK IN PLANK, HOOK THAT CAUGHT ON BEAM CAME LOOSE AND STRUCK HIM IN THE FACE.
Employee has had a history of spinal sterosis w/degenerative disc disease of his neck & underwent a multi-level cervical fusion in 1999. EE has had ongoing problems with pain & numbness, both arms. EE started taking sick leave on 11/04/04. On 12/20/04 we were notified by the state the EE alleges time off was from an incident which occured on 10/04/04 from pulling on a wrench.
EMPLOYEE WAS USING A POCKET TYPE KNIFE TO CUT TIE-WRAPS LOOSE FROM A HEAVY EQUIPMENT FUEL LINE. THE KNIFE SLIPPED THROUGH TIE-WRAP & MADE CONTACT WITH EE'S LEFT ARM SEVERING HIS RADIAL ARTERY. EE HAD SURGERY TO REPAIR ARTERY & WAS RELEASED TO NORMAL DUTIES AS A MECHANIC LEADMAN, NO RESTRICTIONS.
EE WAS REMOVING A PIN FROM SHOVEL LADDER TO MAKEREPAIR TO THE LADDER. WHEN EE KNOCKED PIN OUT OFTHE PUNCH HE WAS USING, IT SHIFTED RESULTING IN LEFT FINGER BEING CAUGHT BETWEEN PUNCH AND LADDER FRAME.
STRIPPING THE JACKET OF 7200 CABLE FOR REPAIR. THE HAND HOLDING THE RAZOR KNIFE SLIPPED AND THE KNIFE CUT THE RING FINGER OF HIS LEFT HAND REQUIRING STITCHES.
EE STATES THAT WHILE TAKING THE END CAP OFF OF THE 140-D MAG. SEPARATOR TO CLEAN OUT MATERIAL BUILDUP USING A PIPE WRENCH, THE CAP BROKE FREE AND EE STRUCK HIS LEFT ELBOW ON THE CORNER OF THE 140-D. MAG SEPARATOR PAN. THIS BECAME REPORTABLE ON AUGUST 8 WHEN EE SOUGHT MEDICAL TREATMENT FOR INFECTED LEFT ELBOW.
HAMMER HIT LEFT THUMB AND SWELLING UNDER NAIL BED OF THUMB REQUIRING RELEASE.
WHILE ATTEMPTING TO REMOVE AN AXLE, EMPLOYEE WAS USING A SLEDGE HAMMER TO KNOCK AXLE LOOSE. A PIECE OF METAL FROM A WELD BROKE OFF & STRUCK HIS RIGHT HAND. REPORTABLE - REMOVAL OF FOREIGN BODY
EMPLOYEE AND CO-WORKER WERE SEPARATING TWO PIECES OF EXHAUST PIPE. WHILE EMPLOYEE WAS HOLDING PIPE, HE MOVED HIS LEFT HAND AND HIS LEFT MIDDLE FINGER WAS STRUCK BY CO-WORKERS HAMMER.
WHILE EE WAS HAMMERING A PIECE OF CONDUIT HE STRUCK RIGHT 5TH FINGER ON A PIECE OF UNIGTRUT CAUSING SMALL CUT. THIS LATER BECOME INFECTED AND WOUND REQUIRED MINOR DEBRIDEMENT AND ANTIBIOTICA O . THIS BECAME REPORTABLE ON 7-15-02 WHEN EE NOTIFIED US OF INCIDENT AND MEDICAL TREATMENT RECEIVED.
