While assisting in the performance of maintenance on a Cat 330 excavator, employee struck in the right jaw area of his face with a glancing blow of a 16 pound sledge hammer. He was holding a metal bar, "with the use of a tie line"; the hammer glanced off the bar and still managed to strike him in the right jaw area of his face.

Employee was assisting in replacing flashing on a conveyor under the primary crusher. He was cutting the flashing with a utility knife when the knife slipped out of the flashing cutting himself across the stomach area.
