Showing all 6Three men were using pry bars to roll over track chain to be installed on EX-650D excavator. When they rolled it over, the pry bar came out of the chain and hit the ground, bouncing back up & striking employee on the front side of his hard hat. He received 1.5" cut on his forehead. This produced minor bleeding from the cut.
Employee was changing a hydraulic hose on a Caterpillar excavator and a pry bar slipped and it struck him in the face.
