Employee was reaming a pin out of a shaft using a hammer and punch. When the pin let loose their hand slid down and got smashed between the punch and steel causing injury.

While tightening/adjusting bolts to level base ring, wrench slipped off of bolt crushing Rt pinky finger between wrench and ring plate.

breaking up frozen mill feed with chipping bar came back and hit self in mouth knocking teeth in.

Tightening bearings on base bolts on the trunion after tightening brought arm back touching kiln causing burn on arm.

The miner was installing a thrust roller in place on #3 kiln. He was using a pry bar to move the thrust roller in place. The load shifted causing the pry bar to suddenly shift and caught in-between his hand and the thrust roller

The employee was sawing a 1' thin walled conduit with a hacksaw. As the saw blade went through the conduit, his finger caught between the conduit end and the hacksaw handle

While cutting ties off of a hose, his hand slipped from oil on hose and cut palm of left hand.

Employee was shoveling from a pile of lime dust & it flushed down & over his boots. He went to clean out his boots & socks & treat his feet with vinegar to neutralize the lime dust. He then put back on his shoes & socks & returned to work. He later put on rubber boots & his feet sweated & the residual dust in his socks caused some burn area to both his feet.

Jacking up the tail pulley framework and the jack slipped and hit the employee in the knee. The knee was cut, however, it did not bleed. The injury was reported and treated with first aid. Two weeks later the area became infected and the employee requested medical attention.

Pulling in the barge rope and he felt something pop in his neck resulting in trapezius strain.

Employee was using a utility knife to open a cardboard box containing parts. As he was holding the box, trying to cut it open, it began to slip out of his hands. When he went to catch it, he pushed the blade of the knife into his lower right leg, causing a laceration.

Employee was cleaning frozen material out of the chute to the screen on the sugar beet plant. The material was covered with snow and frozen together making it impossible to pass through the chute. Employee, using an 8 foot metal bar to break up material, sustained an injury to his left elbow.

Employee states that he was chiseling with a hollow bar frozen chunks out of chute and main screen deck at lower plant for approx. 10 hours and wrists became sore. He requested medical attention on 3-6-07 as he was experiencing burning pain popping in his wrists and wrists & forearm were swollen.

EMPLOYEE WAS USING A WRENCH TO TORQUE A BOLT ON A CYCLONE WHILE WORKING OUT OF AN AERIAL PLATFORM.

EMPLOYEE REPORTED THAT HIS LEFT HAND WAS INJURED WHEN HE WAS HOLDING A SHAFT WITH BOTH HANDS AND A CO-EMPLOYEE MISSED THE SHAFT TO BE HIT WITH A SLEDGE HAMMER AND STRUCK HIS HAND. EMPLOYEE GIV EN MINOR CARE FOR ABRASION AND PLACED NO RESTRICTED DUTY WITH RECHECK AND MMI RELEASE EXPECTED ON 9/10/02.

EE HIT FINGERTIP WITH HAMMER WHILE INSTALLING NOSEBRICK IN KILN

EE STATED HE HAD CUT A SECTION OF PIPE IN THE #1KILN & WHEN FINISHED CUTTING HE BEGAN HAMMERING THE SECTION SO AS TO DISLODGE SOME WHILE HAMMERING THE SECTION SWUNG BACK HITTING THE TO OF THE LEFT FOOT Y

EE WAS PULLING A COMEALONG WHEN HE FELT A STRAININ HIS BACK

EE WAS REPLACING A BELT ON A KILN, WHILE CUTTING WITH A UTILITY KNIFE, EE PULLED THE KNIFE THROUGH AND PUNCTURED HIS CALF LEAVING A LACERATION THAT REQUIRED 3 STITCHES.

EMPLOYEE WAS USING POCKET KNIFE TO SECURE LOCK ON DOOR OF COMPRESSOR.CUT FINGER ON LEFT HAND REQUIRING STITCHES.

EMPLOYEE WAS SWINGING SLEDGE HAMMER &INJURED NECK

CUTTING TAPE OFF MOTOR LEADS, KNIFE SLIPPED & CUT PALM.

PULLED MUSCLES IN BACK WHILE PRYING WITH A SCALING BAR.

HOLDING VICE GRIP ON PIECE OF ANGLE IRON VICE GRIP SLIPPED & EMPL HIT ANGLE IRON WITH HAND.

EMPLOYEE WAS WAKING IN WATER WHILE DIGGING OUT A CONVEYOR BELT. THE WATER REACTED WITH LIME ON HIS SHOES AND LEGS CAUSING BURNS TO HIS LEGS AND FEET.

CUTTING RUBBER HOSE WITH FOLDING KNIFE AND BLADE FOLDED,CUT HIS THUMB REQ 3 STITCHES.

STRAINED BACK WHILE LOOSENING BOLTS.

I NAME LACERATED THE INDEX FINGER OF HIS LEFT HAND WHILE STRIPPING WIRE WITH A UTILITY KNIFE. THE WOUND REQUIRED THREE SUTURES.

EMPL. WAS MOVING, PRYING, ROCK WITH LONG BAR. BAR SLIPPED, HITTING HIM IN SIDE & BREAKING ONE RIB ON HIS RIGHT SIDE. EMPL. WORKED THE NEXT FEW DAYS, & DID NOT TAKE OFF WORK UNTIL 2/14/88.
