Showing all 4Employee was preparing to change a roller on a 345 Track hoe by removing dried mud from the roller and bracket. A dead blow hammer was used to knock off the dried mud. In doing so, the hammer came in contact with the roller and a metal shard come off the roller and punctured the mechanics stomach.
A mechanic was working in the shop on a CAT triple 7 end dump. A mechanic was removing plastic zip ties from a transmission cooler hose with a folding knife with a locking blade. As the mechanic cut the last zip tie the pressured applied carried the knife toward EE's other hand, cutting the glove and EE's hand. The employee received stitches.
THE EE WAS CHANGING A BIT ON A BLAST HOLE DRILL. HE USED A 32" PIPE WRENCH ON HAMMER TO BREAK BIT LOOSE AND HIT HIS HAND WITH THE WRENCH.
