On 2.20.24, EE was pulling & lifting on an empty battery tray on a CAT 349 Excavator trying to remove it. It did not move, so EE used pry bar to remove. EE reported back soreness the next day but did not want to go to doctor. EE requested to see doctor on 2.24.25 & doctor released to return work. On 2.25.25 doctor required physical therapy and allowed EE to return to work.

EE was trouble shooting an issue with the right rear axle. As EE loosened the plug/ bolt pressurized hot oil shot out, hitting EE in face, neck, chest, arm, and stomach. EE suffered 1st and 2nd degree burns.
After splicing a conveyor belt while attempting to cut a trim section with a utility knife lacerated left thumb requiring 6 stitches.

Employee was replacing screens on screen deck. While employee was pressing down on screen cloth with pry bar employee strained his upper back.
Employee was adjusting the position of a tire/wheel assembly with boom hands during tire installation. The tire/wheel assembly was pressed against the rock bar on unit. As EE reached in to move the rock bar out of the way, the tire wheel assembly moved forward "pinching" his right middle finger between the boom hand and rock bar.
While working on bin 24 breaking-up some build-up material, using a chipping bar, his right hand was placed at the end of the bar. a large chunk of material hit the tip of the bar, causing the end with his hand to go up striking an I-beam.
While removing a 70 Lbs chain fall from I beam located 10feet on roof, he went to lift the unit and sustained a hernia on left side of lower abdominal area
While cutting thick rubber conveyor material at the hot mix plant with a utility knife, the knife slipped and cut the employee above his left knee. The employee went to the clinic where his cut was sutured and returned to work the same day without restrictions.
While removing ball drop doors. while using a hammer he hit the fleshy area of his left hand between the thumb and the index finger.
Employee was trying to loosen a bolt, when the bolt broke loose he hit his left knuckle middle finger on a stud.
While removing a slide gate using a come-a-long on Bn-305, he went to move the cable to one side, when he did the cable snapped pinching his index finger on the frame.

While working on ID-360 fan, went to stop the fan from turning, using a pipe wrench. When the wrench got the shaft the wrench also went down pinning the right index finger against the frame of the shaft housing.
Employee was pulling on a wrench to loosen a bolt that was too tight and continued to pull, he felt a strain to right shoulder.
IP put too much tension on a belt wiper. IP decided to relieve some of it and in this process the wrench slipped out of his hand causing it to go 180 degrees around the tensioner and strike him on the right calf.
EE was working with a crew on the primary feeder removing a timing gear. The crew was using a porta-power to pull the gear. While He was pumping on the porta-power pump, one of the bolts on the gear puller broke. As a result, the porta-power ram fell from the feeder striking him on the left arm.
The employee was cleaning material out of a conveyor chute when he caught his finger between his cleaning bar and the frame of the chute.
Employee was using a large wrench to loosen up conveyor belt. The employee then slipped and twisted his back.

EE WAS USING A PIECE OF ANGLE IRON INSTEAD OF A HAMMER TO SECURE THE LID OF A 55 GALLON DRUM. HE STRUCK HIS RT. SMALL FINGER WITH THE ANGLE IRON. THIS RESULTED IN A FRACTURE TO THE TIP OF THE FINGER AND DAMAGE TO THE NAIL.
EE WAS RELEASING THE LOAD BINDER ON A USED OIL TANK WHEN THE BINDER SUDDENLY RELEASED AND STRUCK HIM ON THE RIGHT FOREARM.
EE WAS CLEANING A CHUTE WITH A PICK. AS HE WAS CLEANING A CHUNK OF COMPACT DIRT AND CLAY BROKE FREE AND LANDED ON HIS RIGHT LEG AND FOOT. \
