Hammering on a block, the hammer skipped on the edge and ended up hitting the employee's right hand/wrist. Employee did not seek medical attention until 01/20/2025. With this appointment employee was placed on restrictions of 10lbs push/pull, 10lbs carry/lift with right wrist. Employee came back to work after appointment with brace on wrist.

EE was removing a thermocouple from the tower using a pipe wrench. The pipe wrench slipped off the thermocouple and pinched left pinky between the pipe wrench and the handrail.

Was tightening bolts when one of the wrenches EE was using slipped off the bolt. The bolts were at eye level and the employee was standing while conducting this work. EE was using an adjustable spud wrench and a box end wrench to tighten bolts. Due to the force and angle, the spud wrench slipped and struck them under right eye.

EE was hitting a stiffener ring on a duct with a 16 pound sledge hammer. The hammer glanced off the ring pinching EE's finger between the ring and the hammer handle. EE was wearing gloves.

Employee was swinging sledgehammer to dislodge manhead for Ball Mill, sledgehammer bounced off of manhead. As Employee sought to control the hammer and keep it from falling, they strained a shoulder. Employee did not feel medical attention was necessary until a week later, as discomfort continued. Physician prescribed lifting restrictions and physical therapy.

EE was using heavy pry bar to clean material from clay shredder, when strain occurred. EE did not feel immediately necessary to be evaluated medically. approximately 1 week later, physician diagnosed as thrombosed external hemorrhoid. Procedure performed to drain, employee prescribed restricted duty.

EE was lowering a blastline into a column when his left 4th and 5th fingers became caught between the pulley and the rope. This resulted in lacerations to these fingers. The employee's wounds were immediately treated on site and he was then taken to a clinic to be examined. He was diagnosed with a nail avulsion, given 3 stitches, and a tetanus shot. He was then released to work.

EE was prying on a door and the pry bar slipped. When the bar slipped it pinched his left index finger in between the pry bar and the kiln hood causing a cut to the tip of his finger.

As ee was striking the outside of a chute to clear material inside, the rubberized hammer he was using skidded/slipped off chute, causing employee to strain back. The incident was not reported, the employee continued to work, on 10/18 the employee called in without explanation, and did not work that day. On 10/19, after reporting the incident, physician prescribed limited duty.

EE strained shoulder dislodging buildup with bar. EE requested medical examination approx. 1 1/2 month after incident due to continued discomfort. Physician prescribed anti-inflammatory medication but no restrictions. **EE underwent surgery for continued discomfort on 11/11/10. One lost workday and 60 restricted workdays resulted from this surgery.

Employee was cutting skirt rubber with a utility knife. As he was cutting the rubber, he cut through the rubber faster than he anticipated and the knife slipped and cut his lower right thigh just above the knee.

THE EE WAS REMOVING WOODEN COUPLING PINS FROM COMPRESSOR. THIS JOB REQUIRES DRIVING THE PINS OUT WITH A HAMMER IN TIGHT QUARTERS. HIS HAMMER HIT THE MOTOR NEXT TO THE COMPRESSOR, CREATING A GLANCING BLOW, CAUSING THE HAMMER TO STRIKE THE EE'S LEFT FINGERS.
