EE and a coworker were using a pry bar to dislodge some rocks, slipped, and EE's finger (ring finger on left hand) got smashed in between the pry bar and a metal door. EE applied first aid and returned to work. Went in after work and received 5 stitches.

Showing all 11Replacing bearing on tail pulley of primary crusher. Driving bearing into place when piece of metal broke off and struck employee just above upper lip causing laceration which required two sutures.
Employee was removing lug nugs on stockpile truck tire using breaker bar with etension. Lug nut broke loose suddenly, causing sudden shock to wrist area, resulting in swelling and pain in Left Wrist.
Shoveling hopper out when hand slipped off shovel handle and stuck side of bin, causing pain and bruising in right fifth (pinky) finger. Examination showed no breaks but finger was splinted to protect finger.
Employee was using utility knife to cut rubber hose in order to install new fitting on end of hose. In process of cutting hose, utility knife slipped, causing laceration/puncture wound approximately 1" long on the inside of right thigh about 2"-4" above knee. Wound required 5 stitches.
While tightening the motor mount on crusher, the wrench slipped and employee fell back into belt guard which fell over onto top of left foot.
Employee was helping replace a conveyor belt when the vise grips attached to the conveyor belt EE was pulling on came off of the belt causing EE to fall to the ground on some rocks.
Tightening a bolt on the tailgate of Caterpillar 771D. Pulling down with a wrench he heard something pop in his right shoulder.
Changing bucket tooth on Caterpillar 988F. After removing the pin that holds the tooth, he hit the edge of the tooth with a hammer to loosen the tooth from the adapter when a piece of metal chipped off. The piece of metal struck the right inner thigh of his right leg.
He was putting a hole in plastic with knife, knife didn't lock and cut 2 cm laceration in right index finger.
WHILE PRYING WITH A METAL BAR ON A ROLLER. THE BAR SNAPPED BACK AND HIT HIM IN THE NOSE.
