Maintenance employee was pounding on a bearing assembly with a 16 pound sledge hammer. After the job was completed they noticed pain in right wrist. The next morning their wrist was stiff and sore. The employee did report that their wrist had been sore with pre-existing pain for sometime.

Autoclave operator went to field to change over valves on Geho strainers. A technician was using a wrench on a stuck valve when the valve broke free and threw the wrench, which ricocheted and struck the autoclave operator standing nearby in the face. Fractured nose, facial lacerations, and put off work.

While taking the shoe off of a core tube with a parmalee wrench, employee slipped when the shoe broke free, employee fell to the ground and the parmalee wrench came free and fell on top of employee's left index finger causing a small fracture to the first knuckle.

The employee was pulling on a wrench and the wrench slipped hitting the employee in the bottom lip and front bottom tooth, causing a small laceration to the lip and a small chip to the tooth.

Ee was using a wrench to loosen a bolt, when the bolt came loose the wrench came back and struck the ee in the mouth, fracturing two front teeth. Released to full duty, tooth numbers 8 and 9 will need repair and caps.

Ee was removing a pipe plug from a strainer housing using a pipe wrench. When torqueing on the wrench, EE felt a sharp pain in the low back which worsened throughout the day. Seen at the ER, diagnose with a muscle spasm, and put off work.

Ee was cleaning Geho strainers. After isolation, the strainer housing did not fully depressurize. Ee used a wrench to poke through a layer of mud at the top of the housing, causing the housing to depressurize rapidly, releasing hot slurry that struck the ee on the left side of ee's face, and ee's left wrist. Seen at clinic, diagnosed with 2nd degree burns and released to full duty.

EE was stripping cable with a utility knife. The knife slipped cutting the right thumb. EE seen at clinic, stitches required to close wound, EE released to full duty.

Task was to change leaf springs on a lube truck. While adjusting jack stand height, adjustment tube slipped and caught the right thumb of the tech. Minor laceration/nerve damage.

Ee was removing drain plug with a ratchet and the ratchet slipped, pinching the ee's left pinky finger. Ee seen at ER and sutures were required. Ee released to full duty.

Mechanic working on the rebuild CBD pump split EE's lip. The employee was repacking the pump with a 2 foot bar on the packing pusher to push the packing ring further into the gland. The bar slipped off the gland and the other end of the bar contacted the employees lip. Seen at clinic and stitches were required, released to full duty.

EE was removing flange off pipe with hammer. Struck right thumb with hammer. Seen at clinic, diagnosed with a fracture to the thumb, released to restricted duty.

Ee was bending a piece of metal on trash dumpster for a chain latch when an adjustable wrench ee was using slipped and arm struck the bin, cutting the right forearm. Stitches required to close the wound, ee released to full duty at next shift.

Ee was working with another ee, had swellex in vice and coworker was using wrench to spin it. Injured ee's hand was caught between the vice and the swellex. Seen at ER, diagnosed with fractured fingertip and laceration. Stitches required and ee put off work for 2 days, then restricted.

Contractor employee was repairing a sump. EE cut the discharge hose to length with a utility knife that slipped and made a laceration on left forearm

Two Employees were changing the isolating mounts on the VFD housing on a drill rig and using a come-a-long to lift one of the corners. The come-along broke and the VFD housing dropped down and contacted the socket and ratchet. The ratchet shifted and pinched the employee's index and middle finger in between the ratchet handle and rig substructure.

Ee was aligning a nozzle with hole for pin, struck the inner metal nozzle with a metal hammer, and a metal shard came off the nozzle, striking the operator in the left arm. Ee seen at clinic and released to modified duty.

Ee was replacing cutting edges on a dozer, struck a piece of metal with a 4 lb hammer, and a piece of metal entered right thigh. Seen at clinic, and referred to orthopedist. Determined not to remove metal. Ee placed on restricted duty.

Ee was tightening bolts on a drive line and felt a sharp pain in right shoulder. Seen at clinic and placed on restricted duty.

Ee was removing a coupler from a splined shaft with a hammer. When the ee struck the coupler, a metal shard broke off, striking the ee in the left forearm. The metal had to be removed and stitches were required to close the wound.

Ee was checking air pressure in vehicle tires when EE felt a pop and pain in this right shoulder. EE reported the issue and returned to work. EE asked to see a doctor and was scheduled on 07/01/16. EE was seen on 07/01 and put on restricted duty. Restricted duty began on EE's next scheduled shift of 07/08.

Ee was removing pin, hit pin with a hammer causing metal shard to strike inner left thigh. Ee seen at ER, where the metal shard was removed and the employee was put off work pending follow up on 04/18/16.

Ee was repairing a tooth on a ripper. EE struck the tooth with a metal sledge hammer, causing a piece of metal to break off and enter EE's left thigh. Surgical removal of the metal piece was necessary.

Ee was removing outer insulation on a high voltage cable when the cable slipped and the knife he was using struck his right forearm causing a laceration. Ee was seen at the ER where stitches were required to close the wound. Released to full duty.

When the employee was cutting a plugged hose the knife slipped and cut his hand. The employee went to the ER to have his hand looked at. The ER opened his hand up to see if he had nicked the tendon. The doctor used 3 stitches to close the cut

Employee was cutting ty-wrap holding coils of fiber together with utility knife and slipped cutting back of thumb. Employee had no lost time.

Ee was using a pry bar to adjust a piece of equipment. He felt a tearing sensation and pain in the right abdominal area. He was diagnosed with a torn muscle identified by CT scan and released to restricted duty. Further follow up on 02/23 resulted in the ee being put off work.

Ee was using a utility knife to cut open a box. The ee was not wearing gloves, and the knife slipped, cutting the ee's left thumb. He was seen by a doctor and stitches were required to close the small laceration. Ee was released to full duty.

Ee was loosening a nut on a dual brake can when the wrench slipped and his hand slid down the handle, cutting his hand. He received stitches to close the small laceration. Released to restricted duty.

Ee was removing a filter on a haul truck when he pushed on the wrench, his right shoulder popped. Diagnosed with a strain and put on modified duty which the site cannot accommodate. This employee is a maintenance training student who works a maximum of 3 days per week.

While changing dozer ripper tooth, ee hit pin with 4 lb hammer and piece of metal embedded in left arm near elbow. Originally determined ok to not remove, but orthopedic surgeon recommended removal due to location of metal near joint. Surgery on 07/24/14. Ee full duty until that day.

Ee was using a hook knife while splicing a conveyor belt on the overland conveyor (OLC #2). The knife came through the belt and made contact with his left wrist, causing a laceration to the left wrist. Ee was wearing PPE, kevlar gloves. Ee was evaluated and stitches were required to close the wound. Referred to an orthopedist for evaluation, appt scheduled for 05/12/14.

Cut his thumb while trimming HDPE liner sample using curved utility knife.

Ee was using a knife to strip cable for installation when the knife slipped, cutting the left thumb. Stitches were required to close the laceration. Released to full duty.

Employee was driving picks out of a cutting edge and a piece of metal struck his right cheek. He was seen at the ER where an incision was made to remove the metal. He was released to full duty.

Employee was using a hammer and a piece of metal struck him in the left arm. He was seen at the ER and the metal was removed; stitches required to close laceration. Employee released to full duty.

Employee was cutting a lemon with his retractable knife. Went to put the knife back on his belt and the blade was stuck open. He cut his left middle finger. Was seen at the ER where stitches were required to close the wound. Released to full duty.

Crew was attempting to pull a stuck tube, employee used a parmalay wrench to rotate rods, when he did so the rods dropped pinching his finger between the wrench and rotation guard resulting in an open fracture to the tip of his left little finger.

Emp was breaking bolt loose with a breaker bar and the bolt broke loose. The bar caught the right thumb and bent it backwards. Seen at the ER, diagnosed with a fracture to the right thumb. Cast at the orthopedist, released to restricted duty.

An employee was using an extention on the porta power to remove a pin. The porta power slipped causing the extention to came loose striking employee above the right eye resulting in eight sutures.

When pounding pin with sledge hammer, ee felt pop, burning and pain in elbow. Pain increased over weekend, ee thought this was related to 04/12/12 industrial injury, so he saw Dr. MRI identified triceps tendon rupture, determined a new injury on 09/26/2012. Lost time, surgical repair required.

Employee was stripping a cable and the blade on the razor broke, hitting the right leg with the remaining blade, causing a laceration. Sutures were required to close the wound and the employee was released to full duty.

Ee was retracting a steering cylinder. Thumb was struck by a hammer. Laceration to right index finger required sutures. Ee released to full duty.

Employee changing hose clamps, cutting hose and razor knife slipped. Employee required sutures to left thumb. Released to full duty.

Crew was using SDX 200 tongs to break drill string apart, tongs bound up, helper put hand on the back of tongs to pull to help unbind, tongs came unbound right middle fingertip got pinched between tongs and rotation barrier bushing.

Employee was using an overhead crane and a chain hoist come along, and the chain hit him in the back of the hand. He was seen at the ER, put on restricted duty for two days, use of left hand only. Released to full duty effective 11/15/2011.

Employee was cutting tie wrap with a pocket knife and cut his left wrist. Seen at ER where cut was closed with stitches. Released to full duty.

While trying to remove a screw from the floorboard of a backhoe, the employee felt a pull to the right shoulder. Felt pain that worsened over the remainder of the shift, was seen at ER and orthopedist, diagnosed with a partial rotator cuff tear and sent to physical therapy.

Drill crew was tripping drill pipe out of the hole. Driller assistant lowered the breakout wrench to the table and pinched his finger between the table and the wrench.

Employee loaded shooter with cap - In an attempt to hook lead line employee accidentally shot the shooter injuring his left index finger.
