Employee was using a pipe wrench to disassemble a water pump. EE was pulling down on the wrench from overhead when the jaws slipped causing the handle to hit EE in the forehead. Employee sustained a laceration to the forehead requiring stitches.

Employee was in the process of changing screens. When EE tried to loosen the screen hold down clamp with a pry bar, EE mashed the end of EE's left ring finger on the frame edge, amputating the end of it.
