WHILE ASSISTING ON REPLACEMENT OF THE LOADER BUCKET TEETH, EE REMOVED HIS SAFETY GLASSES TO WIPE THE SWEAT FROM HIS FACE & A PIECE OF METAL BROKE OFF & LOGGED IN HIS RIGHT EYE. METAL BROKE OFF BECAUSE THE HAMMER THE WORKER WAS USING WAS STEEL ON A STEEL TOOTH. EE WENT TO THE HOSPITAL AND HAD SURGERY TO REMOVE THE METAL. HE IS EXPECTED TO MAKE A COMPLETE RECOVERY

EE was cutting 1/2" drip line on the side slope of Leach Pad #4 to remove carbon and scale from emitters. In the process, his knife slipped off the drip line and cut his right leg.
