Showing all 5Employee was assisting change a belt skirting on a conveyor belt, when the cut was being made the boxcutter released from the skirting and due to the force in which employee was pulling it, it came back and hit them in the knee resulting in a laceration to their right knee
Employee was working on the crusher conveyor, helping to change the bearings, while swinging the hammer, employee missed and smashed/cut their left ring finger. Employee was wearing PPE gloves.
Bearing went out on south bag house fan. North bag house was still running causing a draw on the south. Employee tried to stop it with a pipe wrench. Wrench slipped off shaft and struck employee's left arm.
