CLARK AND ANOTHER MINER WERE BARRING DOWN LOOSE ROCK IN A STOPE PRIOR TO BOLTING. CLARK WAS INSPECTING A BARRED DOWN AREA IN THE BACK WHEN A ROCK FROM AN ADJACENT AREA FELL ON HIM, CUTTING TH E BACK OF HIS HEAD. EIGHT STITCHES (STAPLES) WERE REQUIRED TO CLOSE THE WOUND.

EMPLOYEE WAS TIGHENING U-JOINT BOLT ON JCI-125 MUCKER. THE WRENCH SLIPPED, CAUSING HIS RIGHT ELBOW TO STRIKE THE MUCKER FRAME. FIVE STITCHES WERRE REQUIRED TO CLOSE THE RESULTING LACERATION.
