INJURED WAS ATTEMPTING TO LIFT A HYDRAULIC CYLINDER WITH A PRY BAR. AS THE CYLINDER WAS BEING LIFTED, IT SLIIPPED ONTO THE BAR FORCING THE BAR OUT OF HIS HAND SAND ONTO HIS RIGHT FOOT. HE SU FFERED A BROKEN METATARSUS IN HIS RIGHT FOOT. HE WILL BE OFF WORK FOR ABOUT 2 WEEKS.

EMPLOYEE WAS USING A KNIFE TO CUT A PLASTIC LID OFF A 5 GALLON PLASTIC BUCKET. AFTER THE KNIFE BROKE THROUGH THE MATERIAL EMPLOYEE LOST CONTROL OF THE KNIFE & LACERATED THE PALM OF HIS LEFT H AND. HE WENT TO A CLINIC & RECEIVED 4 STUURES THEN RETURNED TO RESTRICTED WORK 10 DAYS.
