EMPLOYEE WAS REMOVING BOLTS FROM A DRIVE LINE IN A HAUL TRUCK. WHILE TRYING TO LOOSEN ONE OF THE BOLTS, THEY FELT SOMETHING PULL IN RIGHT SHOULDER. THEY HAD A SHARP PAIN FOR A SHORT WHILE THEN IT SUBSIDED.

EMPLOYEE HAD OPEN KNIFE IN HAND WHILE ROLLING UP EMULSION HOSE. WHEN THEY WENT TO WRAP THE HOSE, THE KNIFE CUT THEIR LEFT INNER FOREARM ABOVE THE WRIST.
AFTER HAND SCALING, EMPLOYEE REPORTED WRIST IS SORE
Employee was working in J21 and located in an Elliot basket boom that was in an elevated position. They were using a slate bar to scale down loose material on the rib. As they were prying loose a rock on the rib, they felt pain in their abdominal area and then continued working the remainder of the shift.

EMPLOYEE WAS CUTTING CABLE AND CUT LEFT INDEX FINGER
EMPLOYEE WAS CUTTING A ZIP TIE OFF OF A HOSE WHEN THE BOX KNIFE SLIPPED AND CUT EMPLOYEES LEFT HAND BETWEEN THUMB AND INDEX FINGER.
EMPLOYEE WAS CUTTING A TIE WRAP WITH A KNIFE, KNIFE SLIPPED OFF AND CUT ARM.
REMOVING A DRIVESHAFT OUT OF A ST8, PRYBAR SLIPPED, DRIVE SHAFT FELL HITTING EMPLOYEE IN THE MOUTH.
EE CUTTING A SAPLING WITH HAND TRIMMERS WHEN A BRANCH BROKE OFF AND MOVED UP UNDER SAFETY GLASSES AND STRUCK EE IN RIGHT EYE. EE WAS TAKEN FOR MEDICAL EVALUATION. THE LIMB LACERATED THE CORNEA, THE EE WAS GIVEN AN ANTIBIOTIC EYE DROP AND CONTACT LENS FOR PROTECTION WHILE HEALING AND WAS RELEASED BACK TO FULL DUTY WITH NO RESTRICTIONS.
EE CUTTING ANFO HOSE WITH UTILITY KNIFE AND CUT LEFT FOREARM WHICH REQUIRED SUTURES. EE WAS PLACED ON RESTRICTED DUTY AT THAT TIME.
EE splicing a power cable received laceration to left hand/wrist on palm side. EE taken to local ER to have sutures. EE was released from ER back to full duty.
Piece of metal came off of bucket tooth and impacted left leg, causing a laceration and metal fragment imbedded into left leg. EE taken for medical treatment and metal fragment was removed. EE given Tetanus and antibiotic to prevent any infection and EE released back to full duty.
EE WAS LOOSENING A BOLT TO CHANGE THE OIL ON A DRILL WHEN THE EE FELT PAIN IN UPPER RIGHT ARM / SHOULDER. EE WAS EVALUATED BY MEDICAL PERSONNEL AND RELEASED BACK TO NORMAL DUTIES. EE HAS RECENTLY BEEN RE-EVALUATED AND TESTING PERFORMED RESULTING IN A SURGICAL RECOMMENDATION FOR A TORN ROTATOR CUFF. SURGICAL DATE IS PENDING AT THIS TIME. EE REMAINS FULL DUTY UNTIL SURGERY DATE.

EE using a mining bar to pry a loose rock to access a drill hole in the floor and felt pain in right wrist. EE was placed on restricted duty on 10/4/2021.
EE ADJUSTING THE FORKS ON A FORKLIFT WITH A WRENCH, WRENCH HIT EE ON THE CORNER OF THE RIGHT EYE RESULTING IN SCRATCH TO RIGHT EYE. EE RECEIVED PRESCRIPTION EYE DROP AND RELEASED TO FULL DUTY WITH NO RESTRICTIONS.
EE used jack to lift up forklift for service. EE putting blocks under forklift to secure load when the jack mechanically failed and smashed pinky finger on right hand between the board and forklift. EE received 12 sutures and was released back to full duty.
EE PULLING ON A WRENCH TO BREAK LOOSE A BOLT ON A SEAT BELT AND FELT PAIN IN LEFT WRIST. EE TAKEN FOR MEDICAL EVALUATION AND WAS DIAGNOSED WITH A LEFT WRIST SPRAIN. EE PLACED ON RESTRICTED DUTY. EE FOLLOW UP ON MONDAY AND RELEASED BACK TO FULL DUTY.
EE pulling on wrench to tighten boom roller and felt pain in right shoulder. EE was seen for medical attention on 02/01/2021 and placed on restricted duty at that time.
EE testing knife valve, blade came into contact with left index finger. Laceration to left index finger requiring 5 sutures. EE was released back to Full Duty following medical treatment.

EE attempting to loosen a frozen clevise. EE hit clevise with hammer and clevis came loose and hit EE on right forearm. Resulting in fracture to right ulna. EE placed on restricted duty after medical evaluation.
