Employee was installing a termination kit on an electrical cable. EE removed EE's gloves and started slicing the outer jacket of the cable with a razor knife using EE's right hand when the knife slipped contacting the right side of EE's left thumb tip causing a 1cmx1cm avulsion to EE's left thumb. Seen by site medic, referred to ER where EE received restrictions

Showing 50 of 114Employee was tightening a bit in the chuck on a magnetic drill press, the chuck key slipped out of the teeth of the chuck causing EE's third knuckle of right middle finger to come into contact with the edge of metal EE was drilling causing a laceration. EE was seen by the medic and referred to the ER where EE received 2 sutures, no restrictions were issued.
Employee got 6' steel stuck and while trying to get it out, he used a bit knocker to loosen the steel ~ in doing so, he was contacted in the mouth with the bit knocker & chipped his teeth. On 10/19/15, he went to dentist to have them repaired. No Days Lost.

Employee was using a hammer to try to remove the "dog bone" on a haul truck on the front differentials, and it would not move.He stopped hammering & let his left arm go under the dog bone, when it fell on his arm. Xrays were negative, but restrictions were issued. He is working light duty.
