Employee was installing a termination kit on an electrical cable. EE removed EE's gloves and started slicing the outer jacket of the cable with a razor knife using EE's right hand when the knife slipped contacting the right side of EE's left thumb tip causing a 1cmx1cm avulsion to EE's left thumb. Seen by site medic, referred to ER where EE received restrictions

Employee was tightening a bit in the chuck on a magnetic drill press, the chuck key slipped out of the teeth of the chuck causing EE's third knuckle of right middle finger to come into contact with the edge of metal EE was drilling causing a laceration. EE was seen by the medic and referred to the ER where EE received 2 sutures, no restrictions were issued.

Employee got 6' steel stuck and while trying to get it out, he used a bit knocker to loosen the steel ~ in doing so, he was contacted in the mouth with the bit knocker & chipped his teeth. On 10/19/15, he went to dentist to have them repaired. No Days Lost.

Employee was scaling & he let go of scaling bar due to a rock he pried loose fell onto scaling bar. When the scaling bar landed, the end of the bar bounced up & contacted his right knee cap, resulting in laceration which required sutures. Full Release issued. No Days Lost. (This happened on his last shift, then went on days off)

Employee was bending the keeper latch (on a mucker) that holds the air cleaner housing cover in place, while doing so with a pair of pliers, the pliers slipped off the latch resulting in the top of his left hand being struck against the hook on the end of the latch resulting in laceration.

Employee was doing a stope inspection with crew. They were using scaling bars. Employee was barring down on L.side rib while other miner was on R.side of rib.During this time, Employee was struck by the handle end of the scaling bar by other miner.It struck him in back of lower neck causing some discomfort. we took him to ER for xrays, which were negative. Restrictions were issued.

Employee was using a hammer to try to remove the "dog bone" on a haul truck on the front differentials, and it would not move.He stopped hammering & let his left arm go under the dog bone, when it fell on his arm. Xrays were negative, but restrictions were issued. He is working light duty.

Employee was cutting slusher cable and a barb from the cable flew off and went into his hand between the two middle knuckles, like a sliver. We sent him to doctor to make sure nothing was in the hand. Doctor looked & cut into hand to make sure it was clear of a metal sliver. 1 suture was required. He has a full release to return to work.
