Employee was installing a termination kit on an electrical cable. EE removed EE's gloves and started slicing the outer jacket of the cable with a razor knife using EE's right hand when the knife slipped contacting the right side of EE's left thumb tip causing a 1cmx1cm avulsion to EE's left thumb. Seen by site medic, referred to ER where EE received restrictions
Employee was tightening a bit in the chuck on a magnetic drill press, the chuck key slipped out of the teeth of the chuck causing EE's third knuckle of right middle finger to come into contact with the edge of metal EE was drilling causing a laceration. EE was seen by the medic and referred to the ER where EE received 2 sutures, no restrictions were issued.
Employee got 6' steel stuck and while trying to get it out, he used a bit knocker to loosen the steel ~ in doing so, he was contacted in the mouth with the bit knocker & chipped his teeth. On 10/19/15, he went to dentist to have them repaired. No Days Lost.
Employee was scaling & he let go of scaling bar due to a rock he pried loose fell onto scaling bar. When the scaling bar landed, the end of the bar bounced up & contacted his right knee cap, resulting in laceration which required sutures. Full Release issued. No Days Lost. (This happened on his last shift, then went on days off)
At Herztler Project, Sub-Contractor:Great Basin MSHA ID:#L401: A great basin employee was using a retractable utility knife while cutting the pond liner when the knife slipped contacting his other hand resulting in laceration which required sutures. Employee was using a utility knife to cut some materials when the knife slipped & contacted his left hand-palm side causing a laceration. Sutures required. Full Release Issued. No Days Lost.

Employee was attempting to remove pin on clevis (on raise bore head) with a hammer, when he struck himself with the hammer on the left thumb. X-rays revealed fracture to thumb.
The employee seated a split set by hand. He then grabbed a hand tool to further seat the split set and in the process of seating the split set, the split set contacted his hand & caused a laceration.
Employee was bending the keeper latch (on a mucker) that holds the air cleaner housing cover in place, while doing so with a pair of pliers, the pliers slipped off the latch resulting in the top of his left hand being struck against the hook on the end of the latch resulting in laceration.
While breaking a piece of ore sample with a crescent wrench, he struck his Left thumb that was holding the rock, causing a laceration which required 4 sutures & chipped the tip of the thumb bone. No days lost. Full release issued.
Employee was hanging utilities & pipe. He was loosening a nut on a v-clamp & felt pain(discomfort) in his left shoulder. He went to medic onsite. We took him to Doctor. X-rays were negative. However, Doctor issued restrictions.
Employee was doing a stope inspection with crew. They were using scaling bars. Employee was barring down on L.side rib while other miner was on R.side of rib.During this time, Employee was struck by the handle end of the scaling bar by other miner.It struck him in back of lower neck causing some discomfort. we took him to ER for xrays, which were negative. Restrictions were issued.
Employee was using a hammer to try to remove the "dog bone" on a haul truck on the front differentials, and it would not move.He stopped hammering & let his left arm go under the dog bone, when it fell on his arm. Xrays were negative, but restrictions were issued. He is working light duty.
Employee was cutting a band off on a ratchet strap, when the knife slipped & cut his finger through his PPE (gloves) and resulted in a laceration to #2 finger on L. hand. He received 6 sutures. Full release was issued. No days lost.
Employee was hitting a cable cutter with a mine axe, when the mine axe slipped off the cable cutter & his #4 finger on L. hand got between the handle & hand rail resulting in a laceration, which required sutures. Full release was issued. no days lost.

Employee was cutting pipe when a piece of debris flew up & bounced off his right cheek under his safety glasses and into his eye. Doctor issued RX and a full release back to work. No days missed. (This happened on the employees Friday)
Employee was cutting a liner with utility knife. He had left hand on beam, while cutting with his right hand, the utility knife slipped & punctured/cut the palm of his left hand, sutures required. Full release issued. No days missed. This happened on his friday, next scheduled shift is Tuesday 3/15/11.
Employee has driving a bullhorn with a double jack when he struck his left index finger with the double jack, causing a laceration which required sutures. Xrays also showed a small fracture in the tip of index finger. He received a full release.
Employee was cutting tubbing with utility knife, and the knife slipped & cut his wrist, which required sutures. He rec'd a full work release.

Employee was cleaning out the dump gate on a mixer with a short d-handle shovel. The gate closed down on his left hand & his fingers where caught in-between the gate and shovel handle, causing a crushing injury to finger #5.
Employee was cutting slusher cable and a barb from the cable flew off and went into his hand between the two middle knuckles, like a sliver. We sent him to doctor to make sure nothing was in the hand. Doctor looked & cut into hand to make sure it was clear of a metal sliver. 1 suture was required. He has a full release to return to work.
