Showing all 32An air hammer bit got stuck in the wall trying to relieve old concave liners from the primary crusher mainframe. A coworker hit the bit with a hammer, and the bit came free cutting EE's finger and stomach.
Employee was using a hammer while removing a motor fan and EE's hand struck a metal framework with a burr causing a laceration requiring sutures.
Employee was pounding out a roller set from a frame and hit left fifth finger. On 4/10/2017, employee was diagnosed with a fracture and placed on restrictions.

Employee aggravated a pre-existing condition in left hand. EE had surgery this day for trigger finger. EE was using a pry bar to remove an old insulator and felt a pop in hand.
An employee was cutting off an old hose from an air grinder and the utility knife slipped resulting in a cut to EE's left first finger. EE received one stitch.
Employee was cutting plastic cable tie with a utility knife, the knife slipped and punctured the web between the thumb and index finger of the right hand, resulting in a laceration approximately 3/4" long. He received four stitches.
Employee was helping to break up frozen chunks with a bar and pinched his finger between the bar & the chute edge. This resulted in a tuft fracture of the 4th finger, right hand.
Employee was holding air arc torch handle in left hand. With a screwdriver, he attempted to remove screw from handle. The screw was tight therefore, he applied force and the screwdriver slipped off the screw and punctured the web between his left thumb and forefinger. He received four stitches.
Employee was removing pound-down skirting with a 4 lb hammer. He hit the trougher frame with the hammer and his second finger, left hand was between the trougher frame and hammer handle.
The employee was tightening bolts with a rachet and socket on the 34DL04 drill leveling jack pad. The socket slipped and the employee fell backwards catching himself with his left hand on a sharp edged rock. Employee received two stitches.
Employee was pulling a 15" wrench toward him at head level. The wrench slipped and hit him on the left side of the nose.
Using a form clamp spring loaded to remove a concrete from. When ee went to push the clamp on spring was too tight, sprung back, jammed his shoulder.
Employee was cutting bands on a bundle of strap, when the bands were cut and broke free, the employee's hand continued upward and hit the iron rack above.

Employee was securing equipment with chain and chain binder. When trying to lock chain binder, the pipe binder kicked back and hit the injured in the chin.
EE fractured left thumb when the bar EE was using to pry apart two mill rods which were frozen together, slipped when they broke apart and EE fell against one of the rods with hand. EE went to the doctor on 2/04/05 after thumb had swollen up over night. X-rays showed a fracture.
Injured was removing the inner race of a bearing from a crusher counter-shaft housing with a bar and hammer. He was holding the bar and a co-worker was hitting the bar with the hammer. He moved the bar while the co-worker swung the hammer and the bar bounced up striking his left cheek causing a laceration which required sutures.
INJURED WAS STRUCK IN THE STOMACH WITH A PIECE OF METAL WHICH BROKE OFF A LIP LINER ON A SHOVEL BUCKET WHILE BEING KNOCKED LOOSE WITH A SLEDGE HAMMER. THE PIECE NEEDED TO BE SURGICALLY REMOVE D AND REQUIRED STITCHES.
INJURED LACERATED HIS THUMB WHEN HE STRUCK IT WITH A HAMMER WHILE DRIVING A BOLT OUT OF THE CONVEYOR FRAME.
INJURED FRACTURED THE TIP OF HER LEFT THUMB WHEN SHE STRUCK IT WITH A HAMMER WHILE PUTTING ON A FILTER SECTOR CLAMP.

INJURED PINCHED HIS LEFT RING FINGER BETWEEN THE ROTARY DRILL BIT AND THE DRILL BIT WRENCH HE WAS USING WHEN THE WRENCH SLIPPED FROM HIS HAND AND FELL ON HIS FINGER.
INJURED STRAINED HIS SHOULDER AND NECK WHEN THE PIPE WRENCH HE WAS USING SLIPPED CAUSING HIM TO BE JARRED. INJURED DID NOT GOIN TO THE DR UNTIL 6-26. HE AGGRAVATED APREVIOUS OFF THE JOB (CAR A CCIDENT) INJURY WHICH REQUIRED FUSION SURGERY. HE WILL BE SCHEDULED FOR SURGERY ON 7-20-01.
INJURED WAS MOVING A FLOOR HOIST WITH A BAR WHENTHE TIP OF THE BAR BROKE OFF CAUSING A SUDDEN JARRING TO THE RIGHT SHOULDER.
