Nov 5, 2019 Ee was using a bar to clean build-up off of a conveyor frame, the Ee's pinky was pinched between the bar and the handrail. Laceration was cleaned and employee returned to work. Recently, Ee noticed a growth under the skin. Dr recommended that it be removed. Ee had surgery on Feb 2, 2021. Ee lost time due to surgery.

While closing a railcar bottom door, bar slipped and pinched Right ring Finger between bar and rail. Laceration required 3 stitches to close.
EE was cutting wire with a hack saw. After repositioning wire, EE misjudged hand placement and drew blade over Left Thumb causing a laceration that required stitches.
Employee was using a pry bar to unseat and slide out a tram motor. Motor moved, pry bar slipped, and employee's movement carried hands into the frame of the continuous miner frame. Left Middle Finger was pinched between pry bar and miner frame. 2mm was trimmed off the finger tip, 8 sutures were used to close the wound.
EE clearing chunk of material from feed end of screw conveyor. Pry bar being used pushed through chunk and pinched EE's R hand against the chute enclosure. Fractured R wrist & #5 finger. First LWD 08/02
Contract Millwright was cutting zip-ties with a knife. Knife slipped while cutting and cut the left hand between the thumb and index finger. EE received sutures at local emergency clinic. Doctor placed EE on light duty to help keep injury clean for 7-10 days.
EE was using a torque multiplier to install bolts on miner. Tool locked up. EE attempted to reverse setting when tool slipped pinching R index finger against motor. Fracture
Using a slide hammer to remove broken torque shaft, EE pinched L#3 finger between the slide and stopper. Laceration required stitches
EE was driving sprocket from shaft using hammer. Steel chip dislodged from sprocket and lodged in the dorsal aspect of EE's L Knee. Chip was removed, stitches were required to close wound. RTW unrestricted 12/17
Employee was cutting membrane roofing using a utility knife. Holding membrane with left hand, cutting with the right. Utility knife went through membrane and cut employees left hand.
Using pusher bolts to remove miner cutter head end cap, wrench slipped and EE lost balance and rolled L ankle when EE's weight shifted resulting in L ankle fracture
EE tightening flange bolts on a pipeline. Wrench slipped. EE cut R wrist on unidentified sharp object. Stitches required to close wound.
EE prying loose rib near u-G shop, pry bar slipped, EE strained R shoulder. FA treatment applied & EE returned to normal mechanical duties. Subsequent med eval revealed R rotator cuff tear. EE underwent surgery on 2/5/14--first lost-workday.
EE was using a sledge hammer to strike a material chute to dislodge plugged material. Inadvertently EE struck flange causing handle to kick back striking EE's lip. Stitches required

EE was dislodging a sledge used as a spacer for a porta-power that would not release pressure. The hammer ejected striking EE in face damaging teeth. EE RTW but underwent tooth extraction on 11/14.
Channel Lock pliers slipped while being used to remove a latch assembly. EE's L index finger was pinched and cut, requiring stitches
EE was using chain and hoist to turn a gear case in shop setting. Chain caught on case flange and snapped to normal striking EE's L thumb-fracture and laceration requiring sutures. EE was able to return to assigned duties.
EE Burned from Arc Flash when cutting into live DC S.C. Cable for splice. EE locked out & Tagged out wrong circuit.
EE was using a 8lb Sledge Hammer to Dislodge material in loading pocket, pinched Right Index Finger between structure & Hammer handle. - Fractured Finger.
EE's were cleaning a return idler guard using a 4lb hammer. The guard had been removed for cleaning. EE reached to secure the guard, second EE swung the hammer striking 1st EE on the left middle finger causing an open fracture at the first knuckle of the middle finger of the left hand.
EE was attempting to lower a shuttle car onto blocks using a ratchet jack. He lost control of the handle and was struck on the R. ear and face with the jack. EE was unresponsive for a few minutes and then recovered.
EE replacing a hose fitting on #2 Crystallizer. EE needed to cut a short section of hose off. EE placed hose on handrail & used a knife to cut the hose against handrail. EE was in a crouched position, the knife slipped & cut his right thigh. It was cleaned & closed using 8 staples sutures at Carlsbad Medical ER.
A WSI Contract employee was making a splice on the 469 joy car cable & as he was on his knees, he lost his balance while cutting cable. When he fell, the utility blade punctured the left hand. (5 Stitches required) (WSI MSHA ID#6JA) (WSI Contract Employees are supervised by Intrepid Personnel).
A Maintenance worker was struck in the arm by a chip of steel as a co-worker was striking a bit block with a steel hammer.
Swinging hammer - installing bit - broke ring finger
EE was making a belt splice in s810 and was using a utility knife to cut the belt, while doing so the blade slipped off the belt and went up, cutting his right hand wrist area, he recieved 4 stitches.
EE was swinging a 20lb hammer to loosen up some ore that was hung up on the ore pocket. While doing so, he felt a sharp pain in his left shoulder.
Employyee was cutting conveyor belt for splice and was making his final cutting pass. His leg had been out of line of fire but had five inches to six inches left when he shifted his body position that put his leg in the line of fire. The knife slipped off of the conveyor belt and he cut himself above the right knee.
While switching out a rope on a skip, the employee was working on the 3rd thimble. There was mechanics wire tied between the rope and thimble. While under pressure, the mechanics wire broke causing the the bar the employee was holding to smash him between the bar and other linkage fracturing his right wrist.
Employee was changing v-belts on #1 compactor. He was using a pipe wrench to turn pulley while belts were installed. Gear box released pressure and employee's right little finger was caught between pipe wrench and gear box guard. His finger required 5 stiches and a fracture was found.
Employee was hitting the lower B chute gate with a hammer when he hit his right ring finger on a eye that was welded on the chute braking the tip of his finger.
While removing bolts from a flange, co-worker's wrench slipped off bolt striking employee in the mouth. He was holding the back up wrench. Stitches were required in upper lip.
Employee was dumping all hot leach tanks. Dump line on 2B leach tank was plugged. He placed a washout lance in the end of the line & the hot brine water gushed out getting on both legs and left arm. Restricted Duty.
Employee was changing out rail section. He was using a rail hammer to drive spikes on both sides of rail and his lower back began to hurt. he continued to work but pain continued to get worse. Taken to Doctor on 6-7-06. Restricted Duty
During a belt move a piece of belt was wedged between ropes and the stands. While cutting the belt, the utility knife slipped and cut his left hand. Stitches required.
2 employees were pulling a miner trailing cable. 1st employee was using a hot stick to pull. 2nd employee bent down to tie a rope on the cable. The hot stick hook slipped off and hit 2nd employee in the mouth, lacerating the upper lip. 3 stitches were required.
EE WAS TRYING TO BREAK A BOLT ON A COVER OF A SHUTTLE CAR WHEN HE FELT A POP ON HIS RIGHT HAND. RESTRICTED DUTY.
Emp. was pulling conveyor belt around a tail pulley with a rope. The rope broke and the employee fell aprox. 10' to concrete below. Initial accident occurred on 5-16-05. Employee continued to work w/o restrictions.He continued to have pain and was taken to Company Dr. on 6-28-05. X-rays showed a compression fracture to L1 in lower back. Emp. returned to work with restrictions.
Scraping muck off tail end of shuttlecar. Felt something in right eye. Washed eye out, continued to work. Next day went to Doctor and found piece of metal in R. eye.

USING UTILITY RAZOR KNIFE TO CUT PLASTIC FLEX CONDUIT. THE KNIFE SLIPPED, CUTTING HIS LEFT INDEX FINGER. 3 STITCHES REQUIRED
Employee lifting tail piece with step jack - jack slipped and hit employee in the lower lip. Laceration - stitches required.
