Miner was performing maintenance on a hydraulic excavator. A pin was being driven out of the end bearing of a hydraulic cylinder. As the pin was pushed through the cylinder bearing the bar used to push the pin was forced up by the weight of the cylinder and struck the miner in the mouth. Miner sustained injury to lower lip and a tooth.

A CABLE WEDGE WAS BEING DRIVEN OUT OF THE PULL BAIL SOCKET IN WITH A PIN & 10# HAMMER. THE INJURED MINER WAS STRUCK ABOVE THE RIGHT EYE WITH THE HAMMER AS HE LEANED IN TO CHECK THE WEDGE AT THE SAME TIME THE HAMMER GLANCED OFF THE PIN.
