Shop mechanic was changing oil filters on an Cat 775f in the shop. in an effort to loosen the filter with a ratchet wrench, they were pulling on the wrench when it slip off the filter and came toward them hitting them in the right side of the forehead.
Two employees were splicing a conveyor belt. While installing the clamps the wrench employee one was holding was moved and causing the middle finger of left hand to get caught between the wrench and the frame of the conveyor. Medically treated and currently on light duty.
Employee was loosening a bolt on an engine mounting when the wrench slipped off the bolt head. Employee slammed his hand into the engine bracket causing a contusion. Was first treated as first aid only by medical professionals. On later follow-up visit miner was prescribed meds and PT.
Employee was hammering shaft to remove bearing. Sliver of metal broke off shaft and struck employee on the wrist. Sliver embedded 1/8 inch into skin. Sliver was removed by a doctor.
Employee was cutting conveyor to size for installation using straight edge and utility knife. Utility knife slipped off straight edge and cut employee along right index finger of ungloved hand. Cut required 9 stitches to close.
Employee was changing belts on a motor. Employee was using a come-along to pull rubberized motor mount so that belts may be iinserted. Employee attached come-along to eye-bolt on motor housing. Bolt sheared while employee was installing belts. Motor housing pinched employee hand against angle iron causing laceration and fracture.
Employee was clearing brush around electrical cut off boxes with a hand sickle. Felt tightness in his lower back. Felt a great deal of pain. Suffered a muscle strain in his mid to lower back area.
MATERIAL FROZE ON SCREEN CLOTH. USING SHALE BAR TO REMOVE FROZEN MATERIAL AND INJURED RIGHT ROTATOR CUFF.
While clearing blocked chute - rock hit employee's finger.
WHILE ATTEMPTING TO PRY AN OLD BEARING OFF OF FUEL TRUCK, PRYBAR SLIPPED & SPUN IN A CIRCLE & HIT WORKER ON RT. EYEBROW.
3 LB SLEDGE HAMMER STRUCK THUMB, CAUSED LACERATION.
EE WAS TIGHTENING BOLTS WITH RATCHET WRENCH. RATCHET SLIPPED OFF OF BOLT STRIKING HIM ON RT. CHEEK UNDER EYE AT EYE-GLASS LINE. REQUIRED 3 STITCHES.
A TRANSFER CHUTE BETWEEN TWO CONVEYOR BELTS BLOCKED UP WITH ROCK. THE EMPLOYEE WAS TRYING TO UNBLOCK THE CHUTE WITH A STEEL BAR WHEN INSPECTION COVER CLOSED STRIKING HAND.
