SUBJECT WAS PLACING A NEW CHUTE PLATE ON SECONDARY CRUSHER. AS HE WAS TIGHTENING A BOLT, THE WRENCH SLIPPED CAUSING HIM TO LOSE HIS BALANCE FALLING INTO A CATWALK SEVERAL FT BELOW. AS HE FELL HE STRUCK HIS HEAD ON THE RAILING LACERATING HISHEAD.
