On 10/10/16, Supervisor noticed that EE's right eye was red. EE stated that EE did not know what had caused the redness in EE's eye. On 10/12/16, EE went to see an Ophthalmologist who removed a piece of metal from EE's right eye. On 10/14/16, EE told Supervisor that EE was using a hammer to loosen material in a hopper when EE felt something in EE's right eye.
