EE was helping replace drag link on shuttle car. While holding the drag link, the other helper was using a pipe wrench to twist. Wrench slipped off and caught the tip of EE's right index finger between the wrench and drag link. EE went to the ER and was released to come to work the next workday, worked until 01/22/2022 and returned to the doctor and a decision was made for surgery.

EE was using a slate bar to pry the chain guard loose at the #9 Belt Drive. The bar apparently slipped off and struck EE's left shoulder and jaw. EE walked to the working section and reported to the foreman. EE was brought outside and driven to the local hospital for evaluation.

Employee was tapping a bearing onto a drive roller shaft. A small sliver of the bearing chipped off and entered into the left arm, lower bicep area. He went to the ER to have it checked. The doctor removed the small sliver, he is back to work at full capacity.

He was changing a ripper jack on the continuous miner. He was guiding the jack to the alignment holes when the ripper jack slipped and caught his right hand. The chain hoist connected to the jack slipped and allowed the ripper jack to fall.

EE was pulling a chain while using belt clamps. Ee said he had strained his back. EE said it felt like a pulled muscle. EE called back later that day and said he was going to the doctor. EE left mine site without reporting to the Superintendent. EE came back to work on his next sch. shift. EE did not miss any work until 6/4/2010.
