INJURED WAS HAMMERING ON DRILL STEEL (DRILL STEEL BEING USED AS A PUNCH) WHEN PIECE OF SMALL METAL HIT HIM IN RIGHT EYE. WITNESS STATED INJURED WAS WEARING SAFETY GLASSES AT TIME OF ACCIDENT. THIS OCCURRED WHILE REPLACING BITS IN CONTINUOUS MINER MACHINE.

Showing all 3Injured hooked a chain falt to a trolley, he then proceeded to hook a chain to a pipe section. As he put tension on the chain, the trolley slid off of the end of the I-beam, due to a lack of a stop being at the end of the beam; the trolley struck him on the head, knocking off his hard hat, causing a laceration.
Injured and contractor were changing down a tank that was being picked up by contractor. One side of the tank was properly secured and the other was being done when the cheater pipe slipped, allowing the binder to hit injured in the face (near right eye).
