IE & co-worker were inserting a new belt, the choke rigging on the old belt was not going to hold through the process. IE lifted up on the slack of belt & co-worker attempted to cut a hole using the fixed serrated blade on EE's Leatherman. As EE attempted to cut a hole and applied pressure, the blade slipped off and struck IE puncturing the leg.

BACK BEGAN HURTING DURING A TWO WEEK PERIOD FROM THE WEEK OF 2-5-04 TO 2-11-04. AFTER HIS SHIFT ON THE 11TH HE WENT TO A DOCTOR. THE FOLLOWING MONDAU HE CALLED HIS SUPERVISOR AND TOLD HIM HIS BACK WAS HURTING & HE WAS GOING TO TAKE A FEW DAYS OFF. NEVER INDICATED HE WAS INJURED AT WORK UNTIL 3-1-04.
