Showing all 28Employee was using a hammer to remove GET causing a small piece to contact EE's ear. A small piece of foreign body was removed from EE's ear requiring a suture and returned to work full duty.
Driller on LF-90 Core Drill was removing the bit from the core barrel. The driller put a 36" pipe wrench on the bit and pulled on it. When the bit broke loose the pipe wrench slipped off. This caused the driller to fall with the pipe wrench in EE' hand. EE's thumb was pinched between the deck and pipe wrench on impact. The tip of thumb required 6 stitches to repair the damage.
Employee was cutting a zip tie with a knife and cut knuckle. Employee was wearing gloves but sustained a laceration which required 4 sutures.
Electrician was cutting a wire with a box cutter and cut through their glove and sustained a laceration to right index finger. No sutures were required but electrician was released back to work with restrictions of not lifting more than 5lbs while the cut heals.
Technician was removing the belly pan from a 16M motor grader that had lost oil pressure and needed to be towed out of an active mine area. EE was using a pry bar to help dislodge the belly pan when it came down and pinched EE's thumb between the pan and pry bar.

Employee was changing bucket teeth on a track hoe. The employee hit a pin punch with a hammer and the hammer head chipped. The chipped piece struck employee in the right cheek.
Employee put pipe wrench on pipe, the driller rotated the pipe but wrench did not grip, so employee put right hand on pipe wrench jaw to force it to grip the pipe. Pipe gripped for a second then slipped causing the jaw to slam back into employee's right hand and wrist.
EE was pushing on a 36" pipe wrench to tighten a joint of drill pipe.
Employee states he was attempting to cut through a rubber sealing ring that is used on the casing advancer, using a pocket knife, when the knife slipped and cut his left index finger. He was not wearing gloves.
Attemting to remove the drain plug on the front wheel of 274 haul truck using a brake bar. When plug came loose, slipped and pinched finger between bar and rim. Cutting little finger on left hand. (6 stiches).

EE WAS ATTEMPTING TO LOOSEN A BOLT ON A PUMP FLANGE. THE BOLT LOOSENED SUDDENLY AND THE EE'S HAND SLIPPED, STRIKING THE END OF A 1/4" BOLT PROTRUDING FROM THE PUMP MOUNTING BRACKET. THE EE STRUCK THE BACK OF HIS RIGHT HAND, BRUISING IT AND CAUSING A SMALL PUNCTURE WOUND.
