Employee was using a sledge hammer to drive a pin in the bracket for the cat pad on a mule. A piece of the bracket broke off and hit the employee in the left wrist embedding in EE's arm.
Employee was working on the conveyor chain on the continuous miner. Employee hit wrist with a hammer while working on the chain. Employee continued to work until 11/27/2023 when employee was taken off work until employee can see an orthopedic.
Employee was cutting 48 inch mining belt to be used as skirting flaps on the engineered tailpiece. Employee had the belt laying on the shop floor using a straight edge and a razor knife to cut it. The knife slipped and struck the employee on EE's left hand behind EE's thumb causing a laceration.
Crew was using a sledgehammer to cut a block and during the process employee was hit by hammer causing injury to the finger.
Employee was removing spillage from under the belt (roller) with a pick and injured hand.
Employee received a laceration and a fracture to the left thumb when it got caught between the latch mechanism and the release mechanism of a cable come-along while attempting to remove tension from a belt clamp.

While working in the top floor of Grizzly Building, employee received a laceration to left ring finger and had a piece of metal embedded into right hand while attempting to remove a bearing using a 2lb. shop hammer.
Employee was using a come-a-long on longwall face and was cut by a metal burr on a shield while ratcheting the come-a-long
Employee was readying a plunger for drill steel using a utility knife. While attempting to remove the material from the center of the plunger, employee's razor knife slipped causing a laceration to left hand.
Employee was attempting to cut a manilla rope, when employee inadvertently cut employee's hand

Employee was working in an elevated position on a step ladder, utilizing a pipe wrench to tighten fire suppression plumbing, at which time he incurred pain in his left shoulder.
Employee was helping lay rail. While letting the rail down with a road jack, the jack handle slipped out of employee's hands and struck him on right eyebrow
Employee was attempting to retrieve a section of ventilation tubing that was hung with nylon straps to the mine roof. Employee utilized a knife to cut the nylon strap, at which time he also cut his finger.
Employee was using a razor blade type utility knife to cut the plastic bands off a new nylon ratchet strap, and cut his thumb
Employee was using a pick hammer to install a wooden wedge to tighten up a rib board, and struck himself in the mouth with the hammer.

Employee bent over and picked up a "red devil" come-a-long hoist, and incurred pain in his back
Employee was attempting to move wooden crib blocks by swinging/striking/sticking the pointed end of a pick hammer in the end of the wooden blocks, when the hammer glanced off the edge of the wood and struck the employee in the foot.
Employee was using a Simplex mechanical jack to re-rail track haulage equipment and over-exerted himself , causing pain to his neck and shoulder.
Crew was installing additional 4" line to tie into the existing 6" air line, when they were unable to loosen the 2" and the 3" coupler from the ball reducer they started to remove the 6" coupler. As the second bolt was loosened, the coupler blew off striking the employee in the left knee.
Employee was installing plug on welding lead and was using a screw driver to tighten leads. The screw driver slipped and punctured left hand between index and middle finger. Employee continued to work but sought medical attention after his shift when his hand began to swell. Employee is off work being treated for infection.
Employee was building a crib for roof support using 30-inch crib blocks and was using a pick hammer to pull cribs towards him. When doing so the pick hammer glanced off one of the cribs and struck the employee in the lower leg causing injury.
A small piece of rock struck the employee in the left eye when another miner stuck a rock with a sledge hammer to break apart. The employee was wearing safety glasses at the time of the accident. The small piece of rock went in between the side shield of the safety glass frame and struck the left eye and had to be removed by a physician.
Employee was using a slate bar to pry frozen ballast free from the ballast car. He was reaching out with a slate bar when he felt pain in his lower back. The employee finished his shift and sought medical attention after leaving work
EE was in the process of performing maint. adjustments on a roof bolting machine and while using a prybar the EE was struck by the bar, causing a laceration to the EE's right ear.
EE was using a 3 ton chain come-a-long to gather slack chain being generated by the headgate tensioner when the face conveyor flight that was being held in place by the stops broke. This let tension release back into the face conveyor chain, causing the hook on the chain come-a-long to bend & release striking EE on the left lower leg causing injury.
Employee was using a sledge hammer to drive a wood wedge under a locomotive while re-railing. When doing so the ring finger of his left hand was mashed between the hammer handle and the steel step of the locomotive. Injury required sutures as treatment.
EE was removing materials from belt discharge area and while doing this he used a knife to cut a piece of scrap welding cable to remove. While cutting the jacket off this cable, the blade slipped and came in contact with his left thumb, resulting in a laceration to the left thumb. EE was wearing work gloves at the time of the accident.
When cutting insulation back from a 8/3 cable with a razor knife, the blade slipped and came in contact with the index finger of the left hand lacerating the finger requiring three (3) sutures to close and a tetanus shot.
While repairing miner cable, EE was cutting outer jacket off cable holding cable in left hand, using razor knife with right hand. Razor knife slipped off cable and pentrated palm of left hand causing laceration requiring sutures to close. Razor knife safety was review with employee after accident and the with all miners in the weekly safety meeting.
Employee was striking two hammers together while setting posts when one of the hammers chipped off and the metal imbedded into the index finger of his right hand.
Prying rubber seal on junction box with a 4" screwdriver. Hand slipped causing screwdriver to come in contact with right cheek of face that required sutures to close.
Injured was looking upward, pounding wall when something got in his eye.
