EE stated that he was in process of turning the drill carousel with a slate bar when he felt something "pop" in his lower back. He sustained a strained muscle as a result.

The injured and a co-worker were in process of removing plugged coal from the No.7 tail section chute which is part of the raw coal system. While prying with the slate bar to remove the coal, the bar slipped, came up and subsequently struck him on the ear causing a laceration.

An employee of Gil's Contracting stated while using a razor knife (folding style) and cutting a section of rigid water line the knife slipped and struck his left hand, lacerating the top portion of his ring finger.

Employee was helping install a large pin in the bucket door for the 2800 P&H electric shovel. During the process, he was using a 20-pound sledgehammer & while striking the pin, a piece of metal flew off (pin), striking & penetrating his inside upper left leg. The piece of metal was successfully removed at the hospital the same evening; laceration required stitches.

EE and co-workers were in process of working on a 900 Komatsu loader bucket, installing a lip protector. AS he was striking the lip protector with a 16-pound sledge hammer, a piece of metal chipped off from the lip protector, striking and penetrating his left forearm. The piece of metal was successfully removed at the hospital the next morning.

Employee was cleaning out panel seat on coarse refuse screen with a screw driver when his hand slipped and struck a jagged edge on the sub frame. As a result, he lacerated his index finger on his left hand which required four (4) stitches. He was wearing gloves at the time.
