Employee was using a Razor Type knife to cut ties from hoses. While cutting away from body he had his left hand in line of fire. The knife went through the ties and struck his left index finger, resulting in an incision that required 3 stitches.

He was putting pin in track on excavator driving pin hammer with sledge hammer piece on pin flew off hitting him in the leg.

EE & partner were removing wear plates from bed of rock truck. Employee was using an air chisel to clean plates. his partner was using a hammer to knock plates off. While doing this a piece of metal chipped off struck employee over his right eye at the hairline embedding in his scalp. They both were wearing safety glasses and gloves.

Employee was changing teeth on 992G loader bucket (#2366) while hitting teeth with hammer a piece of metal came off of loader and struck him in the chest. It left a small laceration and some bleeding. The foreman was witness and transported to er. x-ray showed no metal present and wound needed 3 stitches. He return to work.

Employee was unplugging chute at quad silo using a slate bar. While prying on the material, it came loose mashing his right hand between the bar and the edge of the chute. This caused a laceration on top of his right index finger between the knuckle and first joint. This required 5 stitches.

Employee was installing a fan pump on a D11R dozer. While using a bar to slide new pump into position the bar slipped and the two outside fingers on right hand were caught between the pump and the bar. Employee finished the job, went home and then went to an emergency room. Both fingers were fractured. They were taped together and wrapped with a bandage.

Employee was cutting plastic tie wrapped around heater hose on a D11 dozer. He was using a pocket knife and the knife slipped cutting his left index finger at the 2nd joint that required 8 stitches.
