IE was prepping whip lines on the longwall face. IE was trying to take a staple out using a pick hammer on the backside of the staple. IE had another hammer tapping on the pick hammer and a piece of metal came off and hit IE in the eye. IE finished the shift and went the next day to have the metal removed.

IE was working in TG-5's CM section. IE was working on replacing bit cones on a continuous miner. IE was holding onto the bit cone when IE was attempting to get the cone started. IE swung the sledge and missed. IE struck the small finger on left hand causing a fracture.

IE was using a pry bar to get the roof mesh aligned better on the roof bolters mesh rack. When IE began prying, IE felt a strain in middle back on right side. IE was diagnosed with a strained back muscle.

IE was observing a vertical pin keeper being driven in on a relay. When a piece of metal came off and struck IE in right arm above the bicep. The piece of metal was not taken out by the hospital. The cut was glued back together.

IE was setting bits on the shear at the end of the shift. IE hit the bit wrench with a hammer. A small piece of metal came off and went into left side of IE's face. Above the check. IE had the metal removed and received two sutures.

IE was working on IE's utv in the TG-4 travel road. IE was in the process of replacing the serpentine belt. IE was using a short flat headed screwdriver prying the belt onto the utv. The screwdriver slipped off and punctured the right index finger on the underneath side of the second knuckle.

IE was swinging a sledge and busting up coal chunks so they could shovel them on to the belt. IE strained their neck and continued to work until 3/18/2023. IE was than taken off work for surgery on their C5-C6 in their neck.

IE was removing broken shear bits from the LW drum with a hammer and chisel. IE was struck in the left forearm by a piece of metal causing a laceration. Medical has determined on 1/10/2022 the IE has a piece of metal in EE's arm scheduled to be removed.

IE was driving "Boomer" rings in to a longwall shield leg with a hammer. IE missed striking the ring and struck right index finger causing a laceration to EE's index finger.

IE was changing a stageloader tensioner. The tensioner was suspended with a come-a-long chain for removal. The IE was holding pressure against the chain when the load shifted. The chain pinched EE's right middle finger causing a laceration requiring stitches.

A group of miners were shoveling the HG-13 xc 30 belt. Miner-1 was leap frogging Miner-2 to continue shoveling down the line. When passing Miner-2, Miner-1 attempted to knock debris from a squeaking top roller (center can) using shovel. The shovel contacted the belt rubber and was knocked from hands causing to strike IE in jaw. This resulted in a fracture to IE's lower jaw.

IE had dropped several 24" ventilation tubes down to reclaim. IE attempted to pry a "T" from the middle. The tubes were tight and when they broke apart the pry bar kicked back striking the IE above the right eye. This caused a laceration requiring 6 stitches.

IE was using a 3lb sledge hammer to set bits in the HG shear drum on the longwall section. A piece of carbide from the bit tip struck IE and embedded in IE's neck.

IE was assisting in pulling the belt back together. A shiv wheel was secured to a chain hanger. While pulling belt slack, the chain hanger broke and the nylon pull rope struck the 4" emulsion line causing it to fall. The emulsion line struck the shoulder and chin of the IE causing a laceration requiring staples.

The injured employee attempted to remove a belt splice pin from a mechanical splice. The employee hooked the splice pin with the claw of the first claw hammer and struck the head of the hammer with a second claw hammer in an attempt to remove the pin. A metal shard from the first claw hammer struck the employee in the arm. The metal shard had to be removed from arm by a physician.

Employee was attempting to trim some skirt rubber from a dump point. Employee used pliers in left hand to bend the skirt rubber back to assist in trimming the skirt. With a utility knife in right hand, employee cut away from body. The knife slipped out of the skirt rubber and lacerated the base of left thumb/hand which was slightly out in front of the knife.

Injured was using come-a-longs on 3/23/11 and heard his shoulder pop. He kept working & came back to work the next day. He was throwing cable over his shoulder on 3/24/11 and it popped again. This is a re-injury to his shoulder from a previous injury that happened on 8/17/09 where he didn't start missing work until 12/2/09. He had surgery and was able to return to work on 6/15/10.

EE was splicing belt on the longwall conveyor belt at Mach mine in IL. As part of the procedure the ee had to cut the belt to make a finger splice, as the ee was cutting the belt with his OLFA razor knife with segmented blades, the ee switched cutting hand from right hand to his left. After switching the ee lost control of the knife causing him to cut his RT index finger.

EE was splicing belt on the longwall conveyor belt at Mach mine in IL. As part of the procedure the ee had to cut the belt to make a finger splice, as the ee was cutting the belt with his OLFA razor knife with segmented blades, the blade broke in the belt causing him to lose control of the OLFA and stabbing him in the upper left forearm.
