Showing all 7Employee was currently receiving physical therapy for a degenerative disk disorder. While working on a shuttle car on 4/23/20, employee felt pain in lower back. Employee continued to work until 4/25/20 when EE elected to seek further medical treatment.
Employee was putting conveyor chain back together on the #16 shuttle car. EE was using a 1 1/2 ton chain ratchet to pull it back together when the hook slipped out of the link. EE was struck on the left ear requiring sutures to close.
