Employee was trying to remove entangled conveyor from pulley on QH331 Cone. Employee was using a pipe wrench to turn the pulley belt, when ee hand slipped catching ee finger in between the pulley and the guard. This resulted in a cut right index finger.

Showing all 3Track roller had come off of Drill Rig. While attempting to reinstall the roller by hitting it with a hammer, a small fragment of metal came off cutting through the employee's glove and cutting EE's left finger and imbedding under skin.
Employee #1 was guiding cheek plates into QJ341, Jaw Crusher. Employee #2 was using a hammer to drive in cheek plate. Employee #2 made a miss swing of the hammer, causing employee #1 to be struck in the forehead. Resulting in a laceration.
