Employee was using a shovel to push a piece of ore. The top edge of the shovel slipped off of the ore and the employee lost balance and fell, jamming arm/shoulder.

03/28/2018 employee stands after sweeping in squatted position and feels pain in back, reports it following day, continues to work and complains it isn't improving on 4/30/2018 so I take them to doctor. At home exercises and muscle relaxants given by doctor, 7 day follow up with little improvement results in work restrictions given on 5/07/2018.

The employee smashed his finger while trying to pry a piece of refractory from a plugged kiln chute. The employee received a broken finger and stitches to the tip of his right ring finger.

The employee was cleaning out the bottom of #1 disintegrator with a long scrapper when a large piece of clay broke free and landed on the tool. the employees finger was smashed between the handle of the scrapper and the metal frame on the outside of the disintegrator.

Employee was installing a braking system on a stretch wrap machine, he was stripping the cabling in the unit. He nicked his left index finger with a utility knife. He did not report it because the cut was small, a few days later he felt it became infected and seeked medical attention. It was determined that the cut had become infected and prescribed medication for treatment.

Diesel mechanic was adjusting the gates underneath the Garwood Trailer when his wrench slipped and hit some mud/dirt that was stuck on trailer knocking it into his face and eyes. Employee was wearing prescription safety glasses at the time of the incident. Medical Treatment, prescription antibiotic given when employee decided to seek treatment 4 days later.
