Clarksville Quarry 1375 · Caught in, under or between a moving and a stationary object

EE was cutting off the inner race to change a bearing on the tail pulley of the screen feed belt. The splatter from the torch was landing directly onto a Chain Jack chain link. The Chain Jack was being used to horizontally suspend the tail pulley assembly. The chain link gave away which immediately caused the tail pulley assembly to release horizontally and pin EE's hand.

Clarksville Quarry 1375 · Struck by falling object

EE was attempting to clear the 7x20 discharge chute when he was struck by several rocks ranging from 4" to 8" in diameter. He was struck on left shoulder & elbow.
