Employee was tightening bolts using a 2 7/8" wrench. The wrench slipped off of the bolt striking the employee above the left temple. Employee had a laceration that was about one inch long.

The employee was pulling an air slide back to gain access between the flanges. Employee was pulling old silicone out before adding new silicone. The chain that was being used to hold the flange slipped, causing the flange to come together on his left index finger.
