Employee was trying to get a gate to move and was tapping it with a hammer and struck the top of his rt index finger.

Employee was tightening bolts using a 2 7/8" wrench. The wrench slipped off of the bolt striking the employee above the left temple. Employee had a laceration that was about one inch long.
