Two employees were driving a shaft thru a gearbox to remove it. The injured employee was holding a pipe while another employee hit the pipe with a hammer to drive the shaft out. The pipe was driven sideways by a bad hammer hit and pinched the employees finger between the pipe and pulley, resulting in a laceration and fracture of the left ring finger.

EE was part of a 3 person team working to remove snow, and ice from a tail pulley. EE was holding the tail pulley from rotating as another employee cleared out. The team leader instructed the EE to stay clear of the conveyor, but the EE did not hear the command. A bar was still in the tail pulley when it was bumped and the bar hit the EE and knocked them to the ground.

Employee was pounding new bearings into a troughing roller can, using a socket and hammer. while pounding, the socket slipped off the bearing and pinched a finger when they swung the hammer again. Employee was wearing gloves.

Employee was using a safety knife to cut hard plastic. It slipped and laceration to left thumb. Stitches to left thumb with lifting restriction of 10 pounds to left arm.

Employee was cutting a piece of torn belting that was wrapped around a head pulley shaft, when the utility knife slipped. He cut the inside of his left wrist.

Employee was exploratory drilling at property adjacent to HM Booneville site. Employee was operating drill connecting continuous flight auger stems. This involves insertion of a metal clip/pin into the connecting points on the auger stems using a hammer. Weight of the hammer is 14 oz ball type. In the back swing motion of hammering employee felt pain in right elbow.

Changing out the dry belts on a motor on the vibrator and while prying up on the motor with a pry bar employee felt a snap in his right shoulder and upper back area. Employee tightened up shortly after and requested doctor visit.

Employee was attempting to clean chute out that feeds plant. Material had built up. While breaking material loose, a piece slid down the side of the chute and struck employee on the knee.
