Employee was using an overhead crane and a chain hoist come along, and the chain hit him in the back of the hand. He was seen at the ER, put on restricted duty for two days, use of left hand only. Released to full duty effective 11/15/2011.

Showing all 8While trying to remove a screw from the floorboard of a backhoe, the employee felt a pull to the right shoulder. Felt pain that worsened over the remainder of the shift, was seen at ER and orthopedist, diagnosed with a partial rotator cuff tear and sent to physical therapy.
While using a folding pocket knife on strap of vent back, knife slipped, cutting left index finger. Received 5 stitches and released to full duty.
While working on a flange on attrition tanks, wrench slipped off and hit employee in mouth, breaking two teeth. Taken to 7 Day Dental where he was treated and released to full duty.
Employee was using a pocket knife with a locking blade and while twisting the knife the lock was released allowing the blade to close, cutting him on the second knuckle of the right middle finger. He was taken to NNRH and received stitches.
Employee's right middle finger was crushed between the tong handle and jaws
Employee was tightening a bolt with a 9/16 end wrench when the wrench slipped off the bolt which caused his elbow to hit a piece of hardware. The next day he was taken to the hospital where it was diagnosed as an RDI, he had a follow up the next day where it was diagnosed as a minor injury. On a follow up visit on the 13th it was diagnosed as an RDI.
While stripping the insulation off of a voltage conductor with a razor knife, the knife slipped off the conductor and sliced through the leather glove and cut his left knuckle.
