9/19/25, EE was straightening metal pipe framework on a drill rig using a chain come-a-long. One of the anchor hooks came loose, striking EE's left hand and thumb. EE was wearing gloves and safety glasses. First aid was administered, and EE returned to work that day. On 10/02/25, EE reported worsening pain in EE's thumb, which was later diagnosed as fractured at Urgent Care.

While trying to remove a metal pin by striking with a hammer, a piece of metal from the pin sheared off and entered the emplyee's hand. It was not until Tuesday Aug 9, 2022 that the metal was removed making the incident reportable. Additionally the employee received 4 sutures on this same date resulting from the metal removal.

Employee was assigned to remove a ball stud on 785C rock truck. Employee was using a 20lb hammer to remove a pin. The employee struck self with the hammer.

Employee was changing a wheel bearing on a service truck. With a pry bar EE was removing the wheel bearing. When the bearing came loose EE fell to the ground. EE caught self with left hand and suffered a laceration and a puncture wound to the left thumb requiring two sutures.

Employee was helping crib the rear of ST768 lube truck so the rear springs could be replaced. The floor jack that was located on the right rear of the truck slipped out allowing the truck to set back down on the floor. The tires were still on the truck but the crib block EE was inserting under the axle kicked out and mashed EE's finger between the crib block and the truck frame.

Employee was using a utility knife to prepare a conveyor belt splice. The knife slipped and struck thigh resulting in a laceration to left thigh.

Employee was cleaning a small ditch out with a long handle shovel. The ditch had already been dug with a small back hoe. EE placed hand between the end of the shovel handle and EE's leg to gain leverage. When EE applied leverage with EE's leg, EE felt pain in the right hand in the area of the pinky finger.

Installing roller on a D-10R dozer, using sledge hammer to knock roller loose, a small sliver of metal clipped off roller, penetrating employee's upper right thigh causing laceration. Note: employee returned back the next scheduled day of work 6-6-2017.

The employee was prying on a guard to retrieve a dropped tool. As EE did, the pry bar slipped allowing the employee to be struck in the mouth by EE's own hand breaking a tooth. On Friday June 3, 2016 it was decide to repair the tooth and repair began. The employee informed the company of the repair work being started on Monday June 6, 2016.
