Work was being done on a conveyor approx 8ft above ground. The work done was overhead for several hours. While replacing a roller the employee felt a pop in EE's right shoulder while prying roller in place. EE immediately reported it at the time. EE let me know 2 wks later that there was still discomfort and received medical evaluation. results: limited use till see specialist.

Showing all 3While removing a flat rock that was wedged in rock box above jaw belt. Employee was swinging down on rock with a sledge hammer when they heard a pop from lower back. Which in turn caused them extreme pain and discomfort when trying to move. Employee was transferred by EMS to hospital.
While swinging a sledge hammer to un choke a rock chute the worker hurt left shoulder on 10/18/2017. On 10/30/17 went to doctor. On 12/4/17 started therapy. On 1/30/2018 first day lost to have surgery.
