Employee was using a slate bar to remove a rock from the Right CM on 006 MMU. While pushing down on the slate bar to slide the rock off the slate bar slipped out of employee's hands causing the bar to hit employee in right eye.

Employee was installing a conveyor motor on a shuttle car. While aligning the motor, the chain ratchet slipped causing motor to hit frame of shuttle car and roll out on employees right foot. Employee was diagnosed with a fracture to the right third toe.

Employee was changing a shut-off valve on a 4" inch water line, the straws that hold the coupling in place were broke so he used a hammer to bust the coupling which was still under pressure and it came apart striking him in the left side of the face causing cuts which required sutures and bruising and a broken orbital bone above the left eye.
Employee was cutting cable with a utility knife when the knife slipped out of the outer jacket of cable insulation cutting the index finger of his left hand just above the first knuckle. He was treated at Hospital at Martin, Ky. he received four sutures and was released and returned to work.
