Employee was using a prybar to open the southeast door to raw mill separator. The prybar slipped causing EE's middle finger to become wedged between the prybar and the platform handrail.

Employee was pulling down with a pipe wrench in right hand, while holding a 1/2 inch pipe fitting with a pair of channel locks in the left hand. The employee heard a pop in the back area of left shoulder while applying downward force with right hand while trying to loosen fitting.

Maintenance employee was performing tracking adjustments on the palletizer belt. The employee had a hammer in right hand and was holding on to the belt frame with left hand. As the employee began to tap on the belt roller with the hammer, the hammer was caught by the moving roller and the employee's upper arm became pinched between the roller and conveyor belt.

Employee was tapping on the seat of a metal tipping gate with a hammer to try and loosen material because the gate would not fully close. During this process the gate closed crushing employee's right index finger against hammer and gate flap.

EE & contractor was working in the flow control gate area. After removing a valve they used a come-along to raise it from the floor to a working surface of just over 2 feet. The ee had his hand positioned on each side of the valve unit as the contractor was removing the come-along from above. The come-along slipped & fell down, landing on the ee's left thumb,causing a fracture.
