On 3/9/2021 the miner was prying a log out of the cooler discharge EE felt discomfort in EE's right lower back. The miner reported this to EE's supervisor and stated that EE didn't want to go and have it checked out. On 3/16/2021 the miner reported to EE's supervisor that the pain had increased over EE's long change off and would like medical attention.

Employee was replacing an A/C blower motor at MCC-3. While removing electrical tape and zip ties with a knife the knife slipped lacerating the employee's left thumb.
Employee was attempting to break up compacted material inside extruder hopper. Employee used a large auger part to strike the side of the hopper. Employee smashed employee's finger between the auger part and the hopper.
At 8:45am EE was remodeling the birdhouse (load out shack). He shot his left index finger with a staple gun.
