Employee was repositioning a hydraulic jack under the right front of a 777-haul truck. While positioning the jack to the correct spot employees left thumb got caught between the jack handle and the tow hitch of the haul truck. Resulting in a fracture and laceration of the left thumb.

Employee was replacing the barrel on 251 pump, and had trouble getting the value removed so EE got a slate bar and began to pry on it, the slate bar slipped mashing the right ring finger, resulting in a fractur to the tip of EE's finger
Employee stated EE was attempting to loosen bolts on a set of scrapers when EE felt pain in EE's left shoulder. This 7000-1 incident report is being submitted under protest only to avoid further enforcement action and in no way constitutes our agreement of the reportability of this incident. We reserve our right to challenge the validity of the citation.
Employee stated had walked up to check on the drill crew when a chain hoist struck EE in groin area. The drillers were attempting to tighten pipe with 2 pipe wrenches and a chain hoist hooked into the wrenches. Pipe wrench broke causing the chain hoist to strike the employee.
Employee stated was using a utility knife to trim a screen panel for the Unit #224 Screen when the knife slipped and EE cut their upper left thigh. The employee received 3 sutures.
Employee stated they had pulled belt through the tailpiece and was going to cut the belt to make a belt splice as they cut the belt they cut their right thumb with a utility knife. Received 5 sutures in the cut on their thumb.
Employee stated had been dropped off to help make a belt splice. Employee stated put a new blade in the knife, and was on knees squaring the belt with a utility knife when the knife came out of the belt and struck EE's right leg just above the knee, causing a 2-3 inch laceration.
Employee stated: was using a small bar to break a lock off a tool box, the bar slipped out from under the lock and struck employee in the lower lip.
Employee stated was changing a tire on the 488 Diesel scoop. EE was using a slate bar to work the tire loose from the studs. As EE was prying, EE felt a pain in the lower back and groin area. The injury was not reported due to unknown if this is pre existing on medical condition based on employee history.
Employee was assisting on a hard bar pin for dozer. Employee was helping to get the pin lined up with the eyes to fit in. Employee was observing while another employee was striking the pin with a hammer. Small piece of metal chipped off the pin and struck the employee in the left knee. Employee will have the metal surgically removed.
Mechanic was laying under Shot loader preparing to remove the rear end EE was prying on the rear end with a pry bay and pulled a muscle in EE's neck and back.
Striking punch with hammer. Missed punch and struck left thumb. Received stitches on left thumb.
Employee was trying to set a bushing in the frame of a miner. Employee stated they held a long handled sledge hammer up against the bushing as another employee would strike injured employees sledge hammer. As the employee swung, they hit the frame of the miner and it deflected and hit injured employees wrist causing a fracture.
Employee stated was using a sledge hammer to knock a final drive out of 406 roof bolter. Upon striking the sprocket a sliver of metal broke off and embedded in the employee's left thigh. Employee sought medical treatment to remove fragment.
Employee stated they were jacking up rail when the jack kicked forward and the bar struck them over the right eyebrow resulting in 3 stitches.
Employee was installing a belt head with a lifting jack when the jack bar came in contact with employees head resulting in loss of consciousness. No lost or restricted work days.
Employee was using a chain ratchet when the hook broke and caused the chain to hit employee in the right elbow resulting in a laceration which required 3 sutures.
Employee stated EE was holding a hammer punch while another employee was using the hammer to strike. The employee swinging the hammer missed hitting the frame on a shuttle car and the hammer bounced back striking the other employee in the left forearm
Employee was trying to turn bottom PLM plug on power center with a wrench when the wrench slipped causing employee's had to smash against the top plug resulting in a laceration that required sutures.
Employee stated EE was attempting to cut a piece of belt when the knife slipped and EE cut left forearm. This resulted in a laceration and sutures to close the wound.
Employee stated EE and another employee were using a slate bar to pry a rock off the skirt board of the tailpiece. When the rock came free, the slate bar slipped, and struck the employee on the inner part of right knee. This resulted in a contusion. The incident became reportable when the employee began missing work on 10/21/16.
Employee stated they were using a come-a-long. The come-a-long was hooked to a roof plate when it fell striking employee on left hand. Employee received medical attention and had to relieve pressure underneath nail bed by puncturing nail.
Employee stated was removing a broken bit from the bit lug. EE was using a punch and hammer to remove the broken bit. EE missed the punch and struck left index and middle finger with the hammer. This resulted in a fracture and two lost work days.
Employee stated EE was using a chain hoist to pull slack in the belt. The hook was attached to the belt rail. When tension was applied, the hook came loose, and the body of the chain hoist struck EE in the mouth. This resulted in the lose of EE's front tooth.
