Employee was trying to recover aqua couplings. Employee was using a crib to remove coupling from waterline and smashed finger between waterline and coupling.

Employee was attempting to couple a diesel locomotive to a lowboy railcar. Employee used a railjack to raise the drawhead of the lowboy, the drawhead fell off the railjack and caught employee's middle and ring fingers on the right hand between the two.

EE was using a utility knife when the knife slipped and cut EE's left middle finger and index finger. EE was not wearing cut resistant gloves as required by company policy. EE stated that EE had just removed EE's gloves to get a better grip. EE received 5 stitches in EE's left middle finger.

EE was assisting Co-Worker who was attempting to remove a cat pad pin, EE was holding a cut off roof bolt that had been modified for this task while Co-Worker was striking it with a 10lb sledgehammer. The hammer, after striking the head of the roof bolt, ricocheted up striking EE in the left eye. This resulted in a laceration above the left eye.

The employee was using a 4 pound sledge hammer to drive a boom pin out of a continuous miner, the miner removed glasses after they fogged up and begin to drive the pin out when a piece of metal came off the pin and embedded into right eye.

Employee was working on putting a conveyor chain together on the company's 310 shuttle car EE was swinging a sledge hammer installing a connector link and felt a sharp pain in EE's stomach EE was diagnosed with a ventral hernia.

Employee stated that while in the process of removing bolts from the scalping screen in the crusher building. EE was using a pick hammer to knock the bolts out of it so they could be removed. In the process of striking the pick hammer, a small splinter of metal flew off of it, and punctured the employees left middle finger.

Employee stated EE and another employee were attempting to make a belt jumper at 3 Head with a piece of extra belt. In the process of squaring up the belt, the knife came out from the belt, and cut the employee in EE's leg. This employee had to receive 4 stitches.

Employee stated EE was working on the spill board on the tail piece and noticed it was spilling between the spill board and the feeder. To fix the gap where the coal was spilling, EE attempted to drive a wedge and missed the wedge and stuck EE's finger with a 16 oz hammer.

Employee stated ee was assisting some other employees to unload a scoop tire from a mantrip. They were using slate bars to unload the tire. In the process of unloading the tire it flipped over into the scoop bucket they were loading it into. This caused the employee to smash finger in between the slate bar and scoop bucket.

Injured employee stated that employee was working on the conveyor chain for #302 shuttle car. Employee stated employee was on knees attempting to pry on the ball valve of the shuttle car so employee could add oil. In the process of prying with a slate bar employee felt a sharp pain in employee's lower back. Employee was placed on restricted duty on 10/17/2017

The injured was using a slate bar to pry a shuttle car tire from the wheel's stud bolts. The slate bar slipped causing hand to strike against the frame of the shuttle car which resulted in a fracture to thumb.

The injured was using a chipping hammer to clean the weld when EE got a piece of slag in eye. The injured was wearing safety glasses with side shields at the time of the incident. EE flushed eye out immediately, but the irritation persisted. EE went to the doctor on 9-15-16 where they removed a small piece of metal.

The injured was cleaning gob out from around the continuous miner bit clip with pick hammer. The hammer glaced off of the bit lug, which caused EE to puncture self in the left hand with the pick end of hammer requiring 3 stitches.
